Youwill be working as part of our amazing multi-disciplinary Team, consisting of Speech and Language Therapists, Community Nurses,Physiotherapists, Occupational Therapists, Vision and Hearing Practitioners and Clinical Support Team.
This post will involve a range of community, office based and home-based working within North Kent. Main duties of the job
You will be working with a caseload of adults aged 18+ with Learning Disabilities. The role will involve providing support and adviceto individuals with a Learning Disability, their family and/or carers, including delivering training where appropriate.
We use both face to face visits, as well as virtual assessments where clinically appropriate. We work in a number of settings,including client's own homes, residential/supported living, day centres or in the community. We also work with multiple agencies andservice providers across Kent, embedding joint working practices to achieve the best outcomes for our locality covers the Dartford, Gravesend, Swanley, Swale and Medway area, which at times may include travelling into rural areasand occasionally across other areas of Kent.

Person Specification
QUALIFICATIONS
Essential
Registered Healthcare Professional with relevant Professional body (HCPC, RCSLT). Degree in a health relevant subject or willingness to complete a degree (within 3 years of appointment).
Desirable
Mentoring qualification equivalent
EXPERIENCE
Essential
Experience of recent community/acute healthcare gained through education or work experience placement or previous employment.
KNOWLEDGE
Essential
Awareness of professional issues and recent developments in the NHS. Knowledge of up to date clinical practice.
SKILLS & ABILITIES
