Job summary

: To providetechnical and administrative support to the Clinical Pharmacy team andClinicians. Facilitate the implementationof medication changes under the guidance of the Clinical Pharmacist and completeaudits around patient safety in the prescribing of medication, sharing thisinformation with the clinical team.

Duties & Responsibilities:

Processrepeat medication requests, including the conversion of acute requests torepeat medicines, changing quantities of medicines Assist withthe Pharmacy team telephone clinic regarding medication queries from patients Issuingprescriptions in line with agreed clinical protocols and within your competency Medicines reconciliation post discharge and on other transfers of care To update and maintain accurate patient medication records on the practice clinical computer system, including advice given and action taken Liaisewith secondary and primary care colleagues to ensure correct medicines arecontinued following the transfer of care Monitorpractice adherence to the repeat prescribing policy To assistthe Pharmacist with data analysis of prescribing behaviour in line with NICEguidance and local clinical commissioning directives Tosupport the Prescribing team by sharing appropriate medication guidance andestablishing a method of communication to both to the Dispensing team andClinical staff To assistwith the development and review of medicines audits Tomaintain and develop medicines communication to staff Tosupport those with chronic diseases to develop good lifestyle changes to assistin the management of their conditions. Provide acontact for patients, Clinicians, Nursing team, Secondary care and Community Pharmacistswith medication queries Project work/prescribing initiatives/clinicalresearch under the support of the Clinical Pharmacist. Liaise with Clinicians and staff to resolve prescribing queries Liaise with nursing and residential homes in the practice area about medication queries.

Communication:

Utilise and demonstrate sensitivecommunication styles, to ensure patients are fully informed and consent totreatment Communicate effectively with patients andcarers, recognising the need for alternative methods of communication toovercome different levels of understanding, cultural background and preferredways of communicating Anticipate barriers to communication andtake action to improve communication Maintain effective communication within thepractice team and with external stakeholders Act as an advocate for patients andcolleagues Ensure awareness of sources of support andguidance ( PALS) and provide information in an acceptable format to allpatients, recognising any difficulties and referring where appropriate.

Delivering a quality service:

Recogniseand work within own competence and professional code of the General PharmaceuticalCouncil Produce accurate, contemporaneous andcomplete records of patient consultation, consistent with legislation, policiesand procedures Prioritise, organise and manage ownworkload in a manner that maintains and promotes quality Deliver care according to nationalguidelines and evidence based care Assess effectiveness of care deliverythrough self and peer review, benchmarking and formal evaluation Initiate and participate in the maintenanceof quality governance systems and processes across the organisation and itsactivities Utilise the audit cycle as a means ofevaluating the quality of the work of self and the team, implementingimprovements where required In partnership with other clinical teams,collaborate on improving the quality of health care responding to local andnational policies and initiatives as appropriate Evaluate patients response to health careprovision and the effectiveness of care Support and participate in shared learningacross the practice and wider organisation Use a structured framework ( root causeanalysis) to manage, review, and identify learning from patient complaints,clinical incidents and near miss events Assess the impact of policy implementationon care delivery Understand and apply legal issues thatsupport the identification of vulnerable and abused children and adults, and beaware of statutory child/vulnerable patients health procedures and localguidance
