The office base is in Andover War Memorial Hospital, we treat patients who live in Andover and the surrounding areas in their own homes. We provide rehabilitation and therapy input for adults with a diverse range of both long- and short-term conditions.

We are a friendly and busy team, which consists of physiotherapists, occupational therapists, and rehab assistants. We work collaboratively with our community nursing team, and colleagues in Hampshire County Council and Hampshire Hospitals Foundation Trust to provide the best care and outcomes for our population.

Experience in front line NHS community therapy is desirable, as is evidence of up-to-date clinical professional development. Applicants need to be keen to develop and apply their clinical skills, have well developed communication, organisational and prioritisation skills and be able to work under pressure. The role is demanding but rewarding, and we are looking for an enthusiastic, motivated and hardworking individual who can work flexibly, efficiently and contribute to the on-going development of the services we provide.

Main duties of the job

The primary role of the Therapist is to provide strong holistic care to the community. To support the Clinical Team Lead (band 7) with day-to-day leadership responsibilities, development and creative leadership.

The Clinical Therapist will work across the caseload, using their clinical skills to identify the needs of patients, triage appropriately and refer/liaise with the correct services.

To provide specialist therapy assessment of patients, using analytical and judgment skills. To provide appropriate patient centred treatment using evidence-based practice where-ever possible. Patients will present with acute or chronic conditions and complex multi-system pathologies neuro, respiratory conditions, orthopaedic rehabilitation, age related deterioration and palliative care.

To devise effective care plans for each patient with specific therapeutic knowledge, recognising him or her as an individual. The plan of care, which has been developed in conjunction with the patient, carer and relevant others, should be outcome based and ensure appropriate pathways of care and communication via liaison and referral to other agencies as required..

The Clinical Therapist will work under the supervision of the Clinical Team Lead receiving specific therapy clinical supervision/ development support from the appropriate leads in the district. The emphasis will be on developing assessment and clinical skills as well as developing skills that can be shared across a wider team.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.
