Main duties of the job

This post will:

Support the functional role of prescribing andmedicines optimisation as a registered and qualified pharmacy technician.

Basic prescribing analysis using ePACT2prescribing data.

Support the implementation of the PCN DES andday-to-day general practice and promote high quality, cost-effectiveprescribing in primary care.

You will need:

Registration with the General Pharmaceutical Council (GPhC), meeting the specific qualification and training requirements specified by the GPhC criteria to register as a Pharmacy Technician Demonstrable and significant experience in primary care, hospital or community pharmacy. Willing to enrol on the CPPE Primary Care Pharmacy Educational Pathway.

Job responsibilities

JOB TITLE: PharmacyTechnician (PCN Medicines Optimisation)

REPORTS TO: ClinicalPharmacist

Job Summary:

To work within the Phoenix Health Group as thePrimary Care Network (PCN) pharmacy technician to support the implementation ofthe PCN DES and day-to-day general practice.

To promote high quality, cost-effective prescribingin primary care, supporting the specifications within the PCN DES, such asStructured Medication Reviews and Medicines Optimisation and others as theycommence and as determined by the PCN.

To undertake specific project work as required bythe PCN.

To support the functional role of prescribing andmedicines optimisation as a registered and qualified pharmacy technician.

Includes bespoke specialist advanced prescribingsupport within the practice.

Basic prescribing analysis using ePACT2 prescribingdata.

Specific operational duties, especially inemergency situations flu epidemics/pandemic (which involves managing thecentralised initial and ongoing allocation of antiviral medicines, vaccines.Ensuring correct storage conditions are maintained at all times, assuringmaximum patient benefit and safety) .

Key Duties and Responsibilities:

Clinicalresponsibilities of the Pharmacy Technician:

undertake patient facing and patient supportingroles to ensure effective medicines use, through shared-decision makingconversations with patients;

carry out medicines optimisation tasks includingeffective medicine administration ( checking inhaler technique), supportingmedication reviews, and medicines reconciliation. Where required, utiliseconsultation skills to work in partnership with patients to ensure they usetheir medicines effectively;

support, as determined by the PCN, medicationreviews and medicines reconciliation for new patients and synchronisingmedicines for patient transfers between care settings and linking with localcommunity pharmacists.

provide specialist expertise, where competent, toaddress both the public health and social care needs of patients, includinglifestyle advice, service information, and help in tackling local healthinequalities;

take a central role in the clinical aspects ofshared care protocols and liaising with specialist pharmacists for more complexpatients;

support initiatives for antimicrobial stewardshipto reduce inappropriate antibiotic prescribing;

assist in the delivery of medicines optimisationand management incentive schemes and patient safety audits;

support the implementation of national prescribingpolicies and guidance within GP practices. This will be achieved throughundertaking clinical audits ( use of antibiotics), supporting qualityimprovement measures and contributing to the Quality and Outcomes Framework andenhanced services.

Technical andAdministrative responsibilities of the Pharmacy Technician:

work with the PCN multi-disciplinary team to ensureefficient medicines optimisation, including implementing efficient ordering andreturn processes, reducing wastage andproducing MAR sheets where needed;

Support practice reception teams in sorting andstreaming general prescription requests, so as to allow GPs and clinicalpharmacists to review the more clinically complex requests;

provide leadership for medicines optimisationsystems, supporting with a range of services to get the best value frommedicines by encouraging and implementing Electronic Prescriptions, safe repeatprescribing systems, and timely monitoring and management of high-riskmedicines;

provide training and support on the legal, safe andsecure handling of medicines;

develop relationships with other pharmacytechnicians, pharmacists and members of the multi-disciplinary team to supportintegration of the pharmacy team across health and social care includingprimary care, community pharmacy, secondary care, and mental health.

Qualifications

Essential

Registration with the General Pharmaceutical Council (GPhC), meeting the specific qualification and training requirements specified by the GPhC criteria to register as a Pharmacy Technician. Evidence of Continuing Professional Development and ongoing commitment. Willing to enrol on the CPPE Primary Care Pharmacy Educational Pathway

Experience

Essential

Demonstrable and significant post qualification experience as a technician in primary care, hospital or community pharmacy. Experience of audit. Working autonomously and as part of a team. Use of clinical IT systems. Competent user of Microsoft Office software.

Desirable

Experience of working within primary care. Experience of project management. Working knowledge of E-pact2 and general GP practice computer systems.

Knowledge and skills

Essential

Excellent communication skills, written and oral with the ability to communicate confidently. Excellent interpersonal skills. Ability to handle sensitive and confidential information.

Desirable

Experience of presenting. Proven record of effective use of networking and negotiation skills.
