We are looking for Paramedic Practitioners to join our Practice Teams across Sussex, to provide a specialist resource for patients within our practice in collaboration with other members of the multidisciplinary team.

Main duties of the job

Seeing patients in the practice and in their own home face to face or remotely via telephone of video consultation. Triage, assessment and treatment of patients with minor illness and injury as well as exacerbations of common long term conditions. Signposting referring patients to specialists in the community, health, social care and voluntary agencies as appropriate. Encouraging patients to take responsibility for managing their own care and treatment where it is safe to do so and provide health education. A holistic approach to patient care, bringing together medical, social and psychological needs. Providing advice and treatment in collaboration with the Duty Doctor where necessary, to manage acute symptoms of chronic long term conditions. Making referrals to secondary care following discussion with the Duty Doctor. Communicating effectively with patients, families, the MDT and wider practice teams.

2.Job Summary

Providea specialist Paramedic resource for health care professionals and serviceusers, working in collaboration with other members of the MultidisciplinaryTeam.

Towork within the community, as an autonomous, accountable, ParamedicPractitioner, in the provision of a holistic approach for individuals includingassessment, diagnosis and treatment, to deliver quality patient services.

Toassess, diagnose, treat, refer or signpost patients/service users who attendsurgery with undifferentiated or undiagnosed condition relating to minorillness or minor injury.

Thepost holder will use advanced clinical skills to provide education to serviceusers, promoting self-care and empower them to make informed choices abouttheir treatment.

3.Key Responsibilities

A. Diagnosingand treating patients presenting with minor illness

Triage and treat patients wishing to see ahealth care professional, making any necessary referrals to other members ofthe primary health care team

Patients with problems needing referral tosecondary care should be discussed with the regular named GP before making suchreferral.

Ensure clinical practice is safe and effectiveand remains within boundaries of competence, and to acknowledge limitations.

Advise patients on general health care and minorailments, with referral to other members of the primary and secondary healthcare team as necessary

Works from the surgery and within communities asan autonomous practitioner caring for patients and families.

Works as an autonomous practitioner, inaccordance with the Health Professions Council. Ensure that personal andprofessional clinical standards are maintained.

To undertake assessment for patients within thecommunity and those attending the surgery, using diagnostic skills andinitiation of investigations where appropriate.

Issue medications as appropriate followingpolicy, patient group directives and local pathways. With independentprescribing when qualified.

To formally and informally impart knowledge andskills to colleagues promoting peer review and best practice within the workenvironment.

To communicate at all levels within the teamensuring an effective service is delivered.

To maintain accurate, contemporaneous healthcarerecords appropriate to the consultation.

Ensure evidenced-based care is delivered at thehighest standards ensuring delivery of high quality patient care.

Works with local policies and procedures.

Enhance own performance through ContinuousProfessional Development, imparting own knowledge and behaviours to meet theneeds of the service.

To achieve and demonstrate agreed standards ofpersonal and professional development in order to meet the needs of theservice.

To participate in the audit process, evaluationand implementing plans and practice change in order to meet patient need.

Where appropriate record relevant clinicalinformation for the management of long term and chronic diseases and themanagement of safe prescribing.

Contribute positively to the effectiveness andefficiency of the team and work colleagues.

B. Pathological specimens and investigatory procedures

Undertakethe collection of pathological specimens including intravenous blood samples,swabs etc. Perform investigatory procedures requested by the GPs

C. Administration and professional responsibilities

Ensure accurate and legible notes of allconsultations and treatments are recorded in the patients notes

Ensure the clinical computer system is kept upto date, with accurate details recorded and amended

Ensure appropriate items of service claims aremade accurately, reporting any problems to the practice administrator

Ensure accurate completion of all necessarydocumentation associated with patient health care and registration with thepractice

Ensure collection and maintenance of statisticalinformation required for regular and ad hoc reports and audit

Attend and participate in practice meetings asrequired

Restocking and maintenance of clinical areas andconsulting rooms as required

5.Flexibility

a) Thisjob description is intended to provide a broad outline of the role. Thepost holder may be required to carry out other duties commensurate with theirbanding and competence.

a) Thepostholder must maintain the confidentiality of information about patients,staff and other health service business and always meet the requirements of theData Protection Act (1998).

The work of Sussex Primary Care is of a confidentialnature and any information gained by the postholder in their role must not becommunicated to other persons except where required in the recognised course ofduty.

11.No Smoking Policy

a) SussexPrimary Care operates a no-smoking policy, in line with Governmentlegislation. This applies to all staff,visitors and patients. It is a conditionof employment for staff that they do not smoke whilst on duty or in uniform oranywhere on Organisation premises. This includes electronic cigarettes.

b) Whilstwe do not discriminate against employing smokers, they are expected to adhereto this policy and all prospective employees should be aware of this.

12.Professional Registration

a) Allemployees who are required to be a member of a professional body are requiredto hold relevant registration and must comply with the standards and guidelinesof their professional practice, education and conduct and must act within theboundary of the code of conduct at all times.
