Job summary

THIS POST IS FIXED TERM/SECONDMENT FOR 6 MONTHS DUE TO MATERNITY COVER.

Applications are welcomed from enthusiastic practitioners who want to make a difference in mental health service provision. You will have excellent communication skills, be self-motivated, proactive and committed to promoting innovation for service delivery.

The successful candidate will support and facilitate the organization of clinical activity, assist in the delivery of group and individual work whilst continuing to support and deliver the clinical activity as required. The post holder will be required to work independently and liaise with various third sector agencies and organizations as well as carers. Excellent skills in multidisciplinary working and the requirement to establish and maintain excellent links with GP practices are essential.

Main duties of the job

Provide support and give time to an allocated group of service users, and promote their recovery supporting their return to a community environment, or supporting them to maintain their place in the community. Support service users to meet their identified needs and outcomes, paying attention to and respect their values, wishes and aspirations to lead their lives in the way they choose, supporting them to access community resources and services, such as housing, benefits,leisure, education and employment.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
