Main duties of the job
Continuing Health Care isstructured across a small number of different work streams that briefly consistof: -
Assessment Teams - includingFast Track assessment work, Funded Nursing Care, and Triage. Ongoing and Review Teams that monitor the wellbeing of individuals and the quality and effectiveness ofcare arrangements that are being funded by CHC, and that respond and adjustcare to changes in need. Appeals, Disputes andRetrospective Assessment Work facilitating meetings with patients, families,or representatives to review non eligible assessment outcomes, leading meetingswith local authority representatives to reach outcome agreements, undertakingassessments for previously unassessed periods of care for people.Asa peripatetic practitioner you may be working in any of the above according toneed at that time, but you will be offered a chance to work in those teamsusually for a minimum of 3-6 months and most often for much longer periods, so youwill get the opportunity to experience different processes and variety of worktasks, be provided with adequate time to become familiar with each work streamand get to work with other nursing, social work, AHP professionals and a range of supportstaff that make up our department.

Job responsibilities
The post holder will play a key role in supporting the Personal Health Commissioning (PHC) service across the region. Working within the remit of the National Framework for Continuing Healthcare, the post holder will deliver an effective and competent level of service and consistently deliver a client-focused service.
The post holder will be responsible for supporting streams of work across our Continuing Health Care Processes and will be assigned to teams that are experiencing the highest demands. These could include working within, Triage/Assessment Teams, Ongoing and Review Teams, or Appeals and Retrospective Assessment Teams.
As a Senior Practitioner your work will be targeted in response to where the service demands might be highest, providing the postholder with a varied caseload.
The post holder will join a supportive, established team of professionals that share the same determination to deliver the most efficient and person centred experience for those people who access this service.
The post holder is expected to have high level of experience assessing & reviewing complex patients with a strong knowledge of the national frameworks.
The post holder will be able to sensitively communicate a consistent and clear clinical view in what can be emotionally challenging and contentious situations.
The post holder must be able to provide and receive highly complex, sensitive or contentious information, negotiate with senior stakeholders on difficult and controversial issues, and present sensitive information to large and influential groups.
The nature of this post will require a reasonable level of physical, emotional and mental resilience, including shifting and balancing priorities and competing agendas, as well as ongoing exposure to direct emotional circumstances such as dealing with relatives / carers.
The post holder should hold a full UK driving licence and have access to a vehicle to be able to fulfil the requirements of this post.
