Job summary

The BLOSM Youth Navigator will work integrally as a member of BLOSM service in part of a multi-disciplinary team. The purpose of this role is to support vulnerable people in Emergency Department (ED) with needs such as, but not exclusive to involvement in physical violence/assaults, exploitation, substance use, social isolation, life skills, housing, criminality and relationships.

As a Navigator, you will be working in ED and community to support children and young people with some of the issues mentioned. Using trauma informed practise, you will need to engage in various forms of communication to support them at a difficult time. You will assist these people and build trusted relationships, getting to know their needs and which service they will be best placed back into the community. It is essential you can communicate with others at all levels, and liaise with colleagues in other hospital departments, statutory and third sector services, to ensure excellent coordination and support for the person you are working with.

The post holder will be employed and accountable to the organisation but at times will work alongside the Violence Reduction Network, Integrated Care Board and other partnerships.

Please note that this role is offered on a Fixed-Term Contract basis, and external secondments will be considered.

Main duties of the job

Provide trauma informed practice to people in ED and follow up in the community in some cases.

Liaise and work alongside other professionals to manage complex cases involving vulnerable children and young people. Maintain communication with the wider MDT when appropriate, these disciplines include Health and Social Care, Police, Education and Voluntary Sectors. Present, attend, influence, and develop relevant training in relation to these complex issues to ensure that the competency needs of healthcare staff and others are addressed. Advocate for the person to support them in working towards their personal goals. Liaise with other professionals to promote health and social wellbeing for individuals. Develop excellent working relationships with departments in the hospital, , Paediatrics, Mental Health Liaison Team, etc. Support people and carers to access appropriate information and support, including sign posting to a range of support services. Experience of discussing ideas and assessments in a professional way, which supports the person in their best interests, communicating their thoughts and wishes. Maintain accurate electronic patient records and other data resources. Attend meetings and feedback as appropriate. Participate in data audit activity aimed at improving the quality of care. Participate in regular staff meeting. Flexibility to work within areas or departments to meet the demands of changing service needs.

QUALIFICATIONS / TRAINING

Essential

5 GCSE's including Maths and English at grade 4 or equivalent Relevant Youth Work qualifications, or experience equivalent Evidence of continued role development and willingness to learn and develop

KNOWLEDGE, EXPERIENCE & EXPERTISE

Essential

Significant experience working with vulnerable children and young people who have complex needs and safeguarding concerns, , Exploitation, Crime Computer literate with good knowledge and understanding of computer packages and databases, , Microsoft Office Have substantial safeguarding experience Understanding of the principles of a trauma informed approach to care and the impact of Adverse Childhood Experiences on young people and adults

Desirable

Demonstrable experience of administrative and organisational experience Knowledge and experience of using Trust IT/patient record/information systems, , EPR, e-referral, etc. Excellent leadership and critical thinking skills
