Job summary

This is an exciting opportunity to join the Adult Eating Disorders Team as a Severe and Enduring Eating Disorder (SEED) Practitioner .

Under the supervision of the Clinical Team Manager and Senior Colleagues, the post holder will take an active clinical role in managing / co-working those clients identified as having a SEED Practitioner where ongoing care relating to maintaining and developing quality of life is required and the chance of full recovery is limited.

The successful candidate must have a Foundation Degree in Health and Social Care / Mental Health at L5 or equivalent experience.

Main duties of the job

Harm minimisation is key to working with clients with SEED, focusing on supporting and sustaining physical and psychological stability, prioritising quality of life, utilizing the new DIALOG+ approach to assessment, planning and implementation of care, whilst harnessing the potential for future recovery.

To work collaboratively with clients and multi agencies involved in care, developing individual maintenance programmes to reduce repeat admissions to both acute and Specialist Eating Disorders Units (SEDU) which have been proven to be largely ineffective in this client group.

Job responsibilities

Responsibilities include:

Being responsible for the assessment, planning, implementation and evaluation of service user care packages, in consultation and with support from the multi-disciplinary team. To work within the context of the DIALOG+, from acceptance into service, through to discharge from the service To contribute to and participate in multi-disciplinary team To be involved in the planning, development, delivery and evaluation of person- centred care planning and person-centred To offer a first line response to referrals via In conjunction with other care professionals, work with the client to meet their needs, as identified through assessment, to include health and social care To work towards and participate in the model of care appropriate to eating disorder services, as stipulated in NICE Guidelines for Eating Disorders. This would include, social inclusion and recovery.

Qualifications

Essential

Foundation Degree in Health and Social Care / Mental Health at L5 or equivalent experience
