Job summary

This is an exciting opportunity to work with a dynamic and supportive team, providing excellent palliative care. The post holder will work predominantly on our inpatient ward at our Pembury site but with the opportunity to work also within our community team and Cottage Hospice. You will be part of our multidisciplinary team and work in collaboration with our medical colleagues and the wider hospice team. The post holder will also support out of hours care as part of an on call rota.

Main duties of the job

Work primarily on the In-Patient Ward, alongside the ST trainee and other Specialty Doctors under the supervision of the Consultant or lead specialty doctor. Admitting and reviewing patients, liaising with other professionals, offering excellent symptom control and End of Life Care.

Work with the Hospice Outreach Service, visiting patients in their homes or outpatient clinics either jointly or alone with the support of the Clinical lead of the day.

Meet patients' families and carers and provide relevant information.

Advise medical and nursing staff in the hospitals and in the community.

Keep thorough and accurate notes on the Electronic Patient Record relevant to patient care.

In-Patient Ward; reviewing every patient daily, ensure each new patient is formally clerked in on the day of admission. Undertake with the nursing team, the monitoring of patients' symptoms and assessment of their clinical needs, supported by the consultant or lead specialty doctor. Arrange discharge medications and write patient discharge summaries on the day of discharge.

About us

Hospice in the Weald provides care across all settings tothe population of Tunbridge Wells and surrounding areas of Kent and EastSussex. In patient services include a 15 bed ward at our Pembury site and a 10bed ward at our Cottage Hospice in Five Ashes. Community services include acommunity palliative care team with dedicated Consultant Nurse, AdvancedClinical practitioners, Clinical Nurse Specialists, and paramedic specialistsoffering support to people in their own homesand outpatient clinics. Inpatient and community care is delivered alongside our Living Well therapies,counselling and support services.

Job responsibilities

Scope & Job Purpose

To provide medical advice and support to the In-Patient Ward, Hospice Outreach Service, Cottage Hospice and Living Well Service as needed. To visit patients at home or in outpatient clinics or remotely as appropriate and to liaise with primary care services as needed. The focus of this post will be working alongside the Consultant on the Ward or lead specialty doctor. There will also be opportunities to work in the Hospice Outreach Service.

To work primarily on the In-Patient Ward, alongside the ST trainee and other Specialty Doctors under the supervision of the Consultant or lead specialty doctor. Admitting and reviewing patients, communicating with patients and those important to them, liaising with other professionals, offering excellent symptom control and End of Life Care. There will also be the opportunity to work with the Hospice Outreach Service, visiting patients in their homes or seeing in outpatient clinics either jointly with one of the Hospice Outreach staff or alone but with the engagement and support of the Clinical lead of the day. To meet the patients' families and carers and assist in their support, providing relevant information as necessary. To advise medical and nursing staff in the hospitals and in the community when requested to do so. To keep thorough and accurate notes on the Electronic Patient Record of all interventions and external contacts relevant to patient care. When working on the In-Patient Ward; reviewing every patient daily and ensuring each new patient is formally clerked in on the day of admission. To undertake along with the nursing team, the monitoring of patients' symptoms and assessment of their clinical needs, supported by the consultant or lead specialty doctor. To arrange discharge medications and to write patient discharge summaries, ensuring they are sent on the day of discharge and ensuring excellence in external communications. To take part in a 1:6 on-call rota including weeknights and weekends. There is always a consultant second on call available for advice. Visits to the In-Patient Ward are occasionally necessary overnight and therefore you should be within 30 minutes travelling time of the Hospice when 1st on call (accommodation can be provided if necessary). At weekends a daily visit to the In-Patient Ward is important as a routine.

Management & Leadership

To contribute to the development of policies and clinical guidelines within the organisation, using an evidence-based approach. To support and supervise the work of the training grade medical staff. To play an active role in and represent the medical team at Hospice wide meetings.

Communications and Teamwork

To strive to achieve excellent verbal and written communication with patients, families, carers, staff and volunteers within and outside the organisation. To be a co-operative and supportive member of the hospice workforce, ensuring that the line manager is aware of any issues with the workload which may affect other members of the hospice team. To participate in the regular medical clinical supervision group and actively reflect on their own practice. To maintain confidentiality of all information acquired especially with regard to patients, carers, staff and volunteers. To respect and guide all volunteers and to develop effective working relationships including, when requested, providing regular feedback on their performance.

Working with Due Diligence

To participate in clinical and other service audits and developments with the aim of ensuring a high standard of patient care and to encourage and support training grade medical staff and other staff undertaking audits. To take an active part in the journal club rota. To be actively involved in research projects, helping to recruit patients for trials undertaken by the hospice.

Training and Development

To take an active role in the education of the multi-disciplinary team including doctors in training, GPs and medical students. To support the consultants in teaching and education of hospital doctors and GPs when requested. To be involved in informal teaching of Ward, Hospice Outreach Service and Cottage Hospice nurses and paramedics. A proactive interest in life-long learning is expected along with engagement in continuing professional development. To attend appropriate and relevant courses and study days both internally and externally as agreed by the line manager. To undertake an annual performance review at the hospice as well as the annual NHS appraisal and through self-development, continuously update and improve knowledge and competencies. To be proactive in undertaking any additional activities required to be revalidated.
