Job summary

An exciting opportunity has arisen to join the Clinical Endoscopist team as a Trainee Clinical initial focus for the trainee will be to attain competency in Upper gastrointestinal diagnostic procedures, however the post-holder will then be supported and receive training in Lower gastrointestinal diagnosticendoscopy once become fully competent in upper GI diagnostic procedures. This post will be based at North Middlesex University Hospital to support the Clinical and Nursing Lead for Gastroenterology and Endoscopy, in meeting and maintaining the standards required by the JointAdvisory Group on Gastrointestinal Endoscopy (JAG).The post holder will be a key member of the Endoscopy team and will work closely with the Advance Clinical Endoscopist, and Clinical Lead for Endoscopy, and prescribing is also an essential component of the role and the candidate will be supported in obtaining this candidate will need to be a registered healthcare professional from a nursing or allied health professional background with previous experience in endoscopy, gastroenterology or an overlapping field of post will be paid at band 6 until all aspects of the essential person specification criteria and job description are met for a Clinical Endoscopist. After which the post will be moved at band 7, while a further Lower GI diagnostic Endoscopy training is commenced.

Main duties of the job

(this must be read in conjunction with the full clinical endoscopist JD)

You will develop into the role of an independent clinical endoscopist performing initially upper gastrointestinal endoscopy procedures/Trans nasal endoscopy with potential further development of skills to include lower gastrointestinal procedures, according to set protocols and guidelines of the Trust and British Society of GastroenterologyTaking part in assessing care needs of patients and referring for further investigations or consultation as appropriate. Participate in clinical research, audit and contribute to the development of knowledge /education in line with JAG, GRS, BSG quality standards and ensure that national, regional and local guidelines are implemented, whilst continually monitoring the quality of-care post is subject to completion of JAG and London academy accredited Clinical Endoscopist Qualification. This must be successfully completed to progress to a band 7 post (within the maximum period of 24 months). Thereafter progress to band 8a once become a non-medical prescriber and completed the LGI training. (please refer to JD for the band 8a clinical endoscopist - attached)

Job responsibilities

Clinical / Endoscopist Role (this must be read in conjunction with the full clinical endoscopist JD) Manage and co-ordinate the care for patients referred for endoscopy. Be responsible and accountable for all elements of professional nursing practice. Safely prepare patients for any investigation and facilitate the patients journey until discharge. Whilst training the candidate will be supported with adhoc and dedicated training lists to enable them to obtain the procedural numbers they will require for independent certification. They will be required to work as part of the Clinical Endoscopist team and therefore undertake shared administration duties such as validation of referrals. The post holder may also have their own telephone clinic for which they will be supported to obtain the necessary underpinning knowledge through shadowing and teaching. Will be expected to have excellent communication skills and be highly motivated and proactive in pursuing learning opportunities whilst being mindful of the shared administration tasks of the wider team and supporting the team in completing these tasks in a timely manner and to a high standard. Once competent the post holder will carry out upper, then eventually lower endoscopy, make diagnoses, take biopsies, and perform polypectomy. They will document their findings using the endoscopic reporting system with both written reports and photographic evidence. Recommendations will be made based on the findings and if relevant, advice given to GP referrers. Comprehensive knowledge of the relevant guidelines will be used to arrange follow-up endoscopy and further investigations. Endoscopic and histological findings will be communicated to patients both verbally and through correspondence and the post holder will also be expected to communicate bad news to patients in a sympathetic and professional manner when there is suspicion of cancer. Draw on a diverse range of knowledge in their decision-making to determine evidence-based therapeutic interventions this may include prescribing medication and actively monitoring the effectiveness oftherapeutic interventions. Actively seek and participate in peer review of their own practice. Appropriately define the boundaries of their practice6. Promote and participate in the implementation of the Gastroenterology Nursing & Midwifery strategy and Core Standards and contribute towards achieving the Trusts Objectives Education To educate patients and their carers to understand the diagnosis, treatment options and their health care needs during and following treatment. To act as a patient advocate in line with the National guidelines and to actively apply in-depth knowledge of holistic/socio-economic and cultural aspects of care to clients within specialty. Participate in teaching and supervision of new and unqualified staff, including medical staff and learner nurses. Advise on the promotion of health and prevention of illness. Maintain own professional/clinical requirements to act as a role model and resource to patients, significant others and staff. Be conversant with current legislation/JAG/NICE guidance for patients requiring gastroenterology care and implement as appropriate. Participate in local/national audits and research relating to Gastroenterology patients. To use information systems to aid regular audit of the clinical role and have an awareness of how information technology can contribute to clinical practice. Lead on audit, quality, and risk management initiatives in supporting gastro enterology services. Utilise nursing research within clinical practice, keep up to date with, interpret and critically evaluate current literature in specialist area and within Professional arena. To ensure evidence-based practice is implemented and updates Trust guidelines to ensure high quality and effective care is delivered. To communicate and update staff on audit/research findings and to initiate any changes to practice, as required. Work with the Clinical and Nursing Lead to agree own learning objectives in order to develop both professionally and personally. To undertake clinical supervision and reflective practice and support other staff to do the same. Develop and maintain a professional portfolio, which complies with NMC recommendation and revalidation. To monitor/maintain a system for monitoring of self about the requirements of professional practice & the maintenance of NMC requirements regarding registration. To provide information and function as a resource for advice to GPs, primary care about patients requiring endoscopy with both written and verbal correspondence. Participate and contribute with staff review and appraisals and support Personal development plans for nursing and allied health professionals within Endoscopy. Act as an assessor/mentor for students and colleagues participating in induction programmes, teaching, developing and assessing competencies and support ongoing learning. Lead on the development and revision of patient information for patients undergoing endoscopy and ensure all information is in line with Trust standards and agreed within Trust Patient Documentation group.
