Job summary

West Kent Primary Care are seeking to employ a Clinical Pharmacist on behalf of Tonbridge Primary Care Network (PCN) to work across one of their five member practices, and alongside their current clinical pharmacy team.

Tonbridge Primary Care Network (PCN)is made up of 5 member GP practices servicing over 50,000 patients - who offerservices ranging from First Contact Physio, Social Prescribers, and ClinicalPharmacy to make a range of services more accessible to our patients.

Main duties of the job

The Practice-based Clinical Pharmacist will improvethe quality and safety of care offered to patients of the Tonbridge. Theywill have regular contact with patients and will reduce the workload of GPprescribers in the practice. They will also support the management of patientswith long-term conditions.

As part of this role the PCN willfund for you to be enrolled into the NHS England CPPE Education Programme whichlasts for 18 months. Study leave and time off to attend courses will be builtinto your work plan.

Job description

Job responsibilities

Work as part of a multidisciplinary team to manage medicines on transfer of care and systems for safer prescribing. Perform face-to-face medication reviews of patients with polypharmacy, especially for those with frailty and/or with multiple co-morbidities and prescribe alternative medications. Undertake face-to-face reviews of patients with specific long-term conditions that fall within their competency (such as asthma reviews). Carry out Improved Access and Extended Hours sessions, including some weekend work. Provide leadership on quality improvement and clinical audit, as well as managing aspects of medicines safety and antibiotic stewardship. Contribute to practice financial stability through optimisation of medicines related targets in QOF delivery. Person Specification

Experience

Essential

Previous experience as a community / acute / primary care based pharmacist Experience in providing clinical advice and expertise on medicines to colleagues and patients

Desirable

Knowledge of structured medication reviews and producing recommendations for prescribing and monitoring Knowledge of medicines management at point of hospital discharge including patients living in care homes Experience of managing long-term conditions in a clinical setting

Qualifications

Essential

University Degree in Pharmacy Sciences Registered with the General Pharmaceutical Council
