Job summary

We need a Social Prescribing Team Manager whois highly motivated to manage, leadand supervise staff of MacmillanSupport Workers - Macmillan Social Prescribers and volunteers to a high standard.

Youwill work holistically to plan, support and help individuals achieve the goalsand outcomes. You will contributeto the ongoing development of team working and implementing of the new service, produce comprehensiveregular outcome focused reports and manage patterns of work ensuring standardsare maintained and improved. You will provide leadership and line management tosupport the delivery of a safe, effective and appropriate service to ensure the teams successful engagement in bespoke care and support plans to addresshealth and wellbeing needs to patients.

Thisis a very exciting time to be involved with Social Prescribing. It has beenidentified as a key component of NHS Englands Universal Personalised Careobjective contained within their Long-Term Plan, as well as Londons HealthInequalities Strategy for healthy communities published by The Mayors Officein 2018. Both strategies recognise the influence of social, emotional, economicand cultural factors on health and well-being and the importance of communityconnections to improving health and wellbeing, self-care and self-management oflong-term conditions.

Main duties of the job

Youwill work as part of a Primary Care Network Multi-Disciplinary Healthcare Team,utilising a holistic, systematic and strength-based approach, and you will be required to providesupport and advice to staff and work closely with Service Manger on complexissues.

You will also have strongleadership and communication qualities, experience of supervising staff andreport writing to support strategic and organisational requirements.

Summary of job:

Working in partnership with MacmillanCancer Support, we aim to develop interventions for people with cancer in thearea and support the delivery of personalised care to everyone diagnosed with cancer.

The role is to manage, lead and supervise staff of MacmillanSupport Workers - Macmillan Social Prescribers and volunteers to a high standard. You will work holistically toplan, support and help individuals achieve the goals and outcomes. Youwill contribute to the ongoing development of team working and implementing ofthenew service, produce comprehensive regular outcomefocused reports and manage patterns of work ensuring standards are maintainedand improved. You will provide leadership and line management to support thedelivery of a safe, effective and appropriate service.

Key tasks and responsibilities:

1. To provide leadership,guidance, supervision and appraisal to the team of MacmillanSupport Workers - Macmillan Social Prescribers and volunteers

2. To support workers indelivering targeted work with individuals to plan their support and achieve thegoals and outcomes that are important to them, and to work with the wider teamto support the development and implementation of an action plan to support theindividual.

3. To make a commitment toregular supervision, including the ability to be responsive to criticalchallenge, advice, feedback and direction.

4. Actas safeguarding lead for the social prescribing service and work according toFamily Actions and City and Hackneys safeguarding policies and procedures,safeguarding vulnerable adults and children in collaboration with statutory andvoluntary agencies, escalating cases when required.

5. Providehigh quality supervision to staff and volunteers, monitor the safeguardingresponsibilities and training of staff and provide support and advice to staffaround safeguarding issues and work closely with Service Manger on complexissues.

6. To establish and maintaineffective liaison with stakeholders including health, voluntary, social andeducation resources, attending relevant meetings as necessary.

7. To work in partnership withall voluntary and community organisations to build a comprehensive database oflocal resources to design and support the Social Prescription Menu and ensure familiarisation of the supportavailable from Macmillan Cancer Support.

8. To ensure information onsources of voluntary and community support is up to date at all times to enableeffective and accurate signposting and linking of individuals with services.

9. To train and develop GPs andprimary care health teams knowledge on how to identify patients suitable forsocial prescribing service referral on a quarterly basis.

10. To monitor outcome tools andanalyse relevant data to support the Service Manager in delivering on KPIs asrequired.

11. Toundertake holistic assessments and co-design a comprehensive Care and Support Planwith individual service users on an exception basis, identifying support needsto ensure maximum engagement in improving health and well-being.

12. Tocarry out case audits on a regular basis, complete audit reports and shareinformation with the SP worker/s and the Hackney services manager.

13. Set up and maintaincomprehensive data and evaluation systems, including individual health outcometool (Wellbeing star and EQ5D).

14. Attend the steering groupincluding representation from all stakeholder groups to support the on-goingdevelopment, monitoring and evaluation of the programme.

15. Provide monthly and quarterlycomprehensive outcome focused reports detailing the progress of the service,including budgets.

16. Tokeep records of your work and adhere to confidentiality, information sharingprotocols and provide monitoring information as required.

17.Toimplement the principles of Family Action and Hackneys Equal OpportunitiesPolicy in every aspect of your work and positively promote the principles ofthe policy amongst colleagues, service users and other members of thecommunity.

18. Towork flexibly as required by the service and to take part in Family Actionsand other organisations meetings and events to promote, support and celebratethe work of the service and the agencies.
