We have an exciting opportunity for a dynamic and motivated pharmacy technician to join a diverse multidisciplinary team as part of our urgent community response and reablement teams.
We are looking for a senior pharmacy technician who has experience in medicines management and of influencing within teams and across the wider multi-disciplinary landscape. The post holder should be enthusiastic and patient focused with a keen interest in safe use of medicines. The role offers flexibility to plan and manage a patient caseload and work with a team of pharmacy technicians to meet the needs and goals of the service. The role will involve regular travel across the local area within Lambeth and Southwark; the post holder must be able to meet the travel requirements of this post.

Main duties of the job
To provide safe, efficient, high quality medicines management support to the Integrated Local Services (ILS) directorate, working closely with Integrated Care Pharmacists to deliver and develop the pharmacy service in accordance with the objectives set by the Lead Pharmacist Integrated Local Services (ILS).
The post holder will be involved in the provision of domiciliary medicines management and optimisation pharmacy services within ILS.
This role requires a commitment to patient centred care and will involve undertaking medicines reconciliation, assessing medicines support needs, health promotion activities, providing information and advice.
To work with the multidisciplinary team (MDT), including Adult Social Care to provide holistic care to patients in their own homes as part of the urgent community response and reablement service.
To provide high quality clinical advice to patients and healthcare professionals on medication issues promoting patient centred care and evidence-based medicine.
The post holder will also be expected to undertake clinical observations and identify patients at risk in their homes.
To contribute to the training and supervision of allied health professionals, nurses and reablement workers in medicines reconciliation and identifying medication support needs assessments.
To manage medicines stock held on community sites.

Integrated Local Services provide a range of community and inpatient services for the communities of Lambeth and Southwark. Services provided include neighbourhood nursing, specialist rehabilitation & reablement, specialist palliative & end of life care, vulnerable adults and prevention services, and the @Home service.
Services are delivered in various locations health centres, community bed-based units, and patients' homes. We work closely with teams at King's College Hospital and with social, mental health and voluntary sectors.
Our aim is to enable people to live healthy independent lives for as long as possible within their normal place of residence.
Through close multi-disciplinary relationships and new ways of working, the pharmacy team enables the service to make best use of capacity to help meet demand as well as strengthen existing relations with Adult Social Care.
You will join a team of experienced pharmacists and pharmacy technicians within GSTT who are driving medicines optimisation across Lambeth and Southwark.
You will work closely with multidisciplinary teams including community matrons, GPs, therapists, consultants, community pharmacists as well as other specialist and generalist healthcare and social care practitioners to optimise medicines use.

BTEC Pharmaceutical Science/NVQ Level 3 or GPhC accredited equivalent Accuracy Checking Pharmacy Technician (ACPT) qualification LPET Technician Self Development Programme- Stage 2, AIMS, MOP from HEE LaSE or equivalent Medicine Management qualification Registration as a Pharmacy Technician with the General Pharmaceutical Council (GPhC) Mandatory professional CPD as defined by General Pharmaceutical Council.
