Main duties of the job

We are looking for an experienced pharmacist to join our team of band 7 pharmacists who is forward-thinking and dynamic and able to lead, manage and co-ordinate the work of the pharmacy team on the admissions ward to ensure the provision of a high quality, cost-effective, safe and responsive service to acutely admitted medical patients treated for a variety of conditions.

As the admissions pharmacist, you will work as a member of the acute medicine team in the Trust and assist in all aspects of medicines governance ensuring risks are managed and best practice is achieved.

Yeovil District Hospital NHS Foundation Trust merged with Somerset NHS Foundation Trust in April 2023 and as a combined Trust we provide clinical pharmacy services to two acute hospitals, specialist mental health units and community hospitals across Somerset. Job responsibilities

Provide a comprehensive clinical pharmacy service as an integrated member of the acute medicine team at Somerset NHS Foundation Trust (FT).

Ensure a high level of clinical pharmacy care and expertise for acutely admitted medical patients treated for a variety of conditions.

Support clinical governance processes, such as incident reporting and investigation, complaint handling, development of guidelines, policies and procedures, and the monitoring and evaluation of services.

Work as a member of the multidisciplinary team to provide assurance to Somerset NHS FT on the safe, effective and efficient use of medicines for acute medical patients.

Work as an independent prescriber, in line with competencies, as a member of the medical and non-medical prescribing teams in acute medicine, contributing to medicines optimisation and ensuring treatment is evidence based and patient focused.

Support and work alongside Physician Associates within the acute medicine team to ensure the timely and appropriate prescribing of medicines.

Work closely with the acute medicine team based in the Emergency Department to promote and support accurate drug history taking and medicines reconciliation.

Ensure all patients admitted to the Emergency Admissions Unit have received an appropriate pharmaceutical assessment and that all such information and any resulting actions are transferred with the patient on discharge or transfer from the unit.

Extend the boundaries of traditional professional clinical pharmacy practice for acutely admitted medical patients to enhance patient care and positively impact on the patient experience.

Support the development and implementation of ePMA on acute admission units at YDH.

Act as a role model to staff, and support their learning and development

Support the teaching and education of pharmacy and other multidisciplinary staff groups on medicines-related matters relevant to acutely admitted medical patients

Provide general high quality clinical pharmacy services as a member of the clinical pharmacy team meeting the needs of patients and healthcare professionals within the Trust.

Person Specification

Qualifications

Essential

Masters degree or equivalent in Pharmacy Full current registration with the General Pharmaceutical Council (GPhC) Postgraduate Diploma or MSc in Clinical Pharmacy Registered independent prescriber or willing to undertake training.

Experience

Essential

Demonstrable hospital pharmacy experience delivering clinical pharmacy practice at an advanced level. Demonstrable clinical pharmacy experience of general medicine, general surgery (including non-specialist service provision to acute and emergency patients) Significant experience in medicines management of patients with complex medical conditions and polypharmacy.

Desirable

Experience in clinical training and/or mentoring of pharmacy staff. Experience of staff appraisal and objective setting.
