Job summary

We are seeking a Pharmacy Technician who is patient focused and passionate about Primary Care to join our Pharmacy Team, the role can be either based fully in practice or a hybrid role with a minimum requirement to be in the practices 3 days per week.

You may or may not have already worked Primary Care but you will be able to demonstrate the difference you can make to our patients and have the desire to work with our colleagues to make our pharmacy services even better.

Main duties of the job

The role will support GP practices and the wider PrimaryCare Networks to ensure the delivery of safe, effective and efficient systemsfor medicines related clinical correspondence; repeat prescribing, medicinesoptimisation, reducing medicines waste and maximising patient outcomes.

The role is based within our Ripon and Masham PCN which comprises 4 practices 3 within the heart of Ripon and 1 based in our rural setting at Masham.

Job responsibilities

The following describes the key responsibilitiesthat the postholder will deliver and/or work towards delivering aftersupported ongoing development.

Undertakepatient facing and patient supporting roles to ensure effective medicines use,through shared-decision making conversations with patients. Carry outmedicines optimisation tasks including effective medicine administration (checking inhaler technique), supporting medication reviews, and medicinesreconciliation. Where required, utilise consultation skills to work inpartnership with patients to ensure they use their medicines effectively. Support,as determined by the PCN, medication reviews and medicines reconciliation fornew care home patients and synchronising medicines for patient transfersbetween care settings and linking with local community pharmacists. Providespecialist expertise, where competent, to address both the public health andsocial care needs of patients, including lifestyle advice, service information,and help in tackling local health inequalities. Take acentral role in the clinical aspects of shared care protocols and liaising withspecialist pharmacists for more complex patients. Supportinitiatives for antimicrobial stewardship to reduce inappropriate antibioticprescribing. Assist inthe delivery of medicines optimisation and management incentive schemes andpatient safety audits. Supportthe implementation of national prescribing policies and guidance within GPpractices, care homes and other primary care settings. This will be achievedthrough undertaking clinical audits ( use of antibiotics), supportingquality improvement measures and contributing to the Quality and OutcomesFramework and enhanced services. Work withthe PCN multi-disciplinary team to ensure efficient medicines optimisation,including implementing efficient ordering and return processes, and reducingwastage. Provide supportfor medicines optimisation systems across PCNs, supporting practices with arange of services to get the best value from medicines by encouraging andimplementing Electronic Prescriptions, safe repeat prescribing systems, andtimely monitoring and management of high-risk medicines. Providesupport on the legal, safe and secure handling of medicines, including theimplementation of the Electronic Prescription Service (EPS). Provide support to general practice staff andpatients with regards to prescription and medication queries. Assist in thedevelopment and improvement of repeat prescribing & medicinesreconciliation processes and systems for safer prescribing, providing expertisein clinical medicines advice while addressing both public and social care needsof patient. Qualifications

Essential

Mandatory registration with the General Pharmaceutical Council. NVQ Level 3 in Pharmaceutical Services.

Experience

Essential

Previous work experience in a pharmacy or healthcare setting. Competent user of Microsoft Office programmes, including Outlook, Excel and Word to create simple plans and reports. Ability to maintain effective working relationships and promote collaborative working.

Desirable

Previous work experience in a primary care setting or GP practice. Experience with using SystmOne clinical system.
