Job summary

Main duties of the job

The post holder will be expected to:

Complete the CPPE Development Pathway Perform structured medication reviews (SMRs) to proactively manage patients with complex polypharmacy and care home residents Be involved in the management of long-term conditions Embrace service development to grow their capabilities and the clinical pharmacy team

The ideal candidate will be a Pharmacist with clinical experience and being an independent prescriber is desirable. We are looking for someone who is committed to improving patient care and is passionate about clinical pharmacy. The candidate must be able to work effectively as part of the practice team and be comfortable in encouraging new ways of working. Whilst prior experience in General Practice would be an advantage, pharmacists from other sectors with transferable skills are very welcome.

14,500 patient population in the market town of Melksham, Wiltshire. Our local PCN consists of three GP practices: Bradford on Avon & Melksham Health Partnership, Giffords Surgery and Spa Medical Practice together we cover a population of ,000 residents across the towns of Bradford on Avon and Melksham Innovative and efficient same day appointment care navigation and hub triage system - offering online, phone and F2F contacts. Majority of patient requests and queries submitted through our highly successful new econsult online platform. Purpose built spacious modern surgery with plenty of parking, 20 mins from Bath. Large multi-disciplinary team including GPs, ANPs, Mental Health Specialist Nurse Paramedics, Clinical Pharmacists, GP Assistants, First Contact Physiotherapists, Social Prescribers, a dedicated Older Persons Nursing team, GP and PA trainees. Job responsibilities

Shape the future direction of Clinical Pharmacy in primary care

The ideal candidate will be a Pharmacist with significant clinical andquality improvement experience and be an Independent Prescriber. We are lookingfor an individual with leadership qualities, who is committed to improvingpatient care and is passionate about clinical pharmacy in Primary Care. Thecandidate must be able to work effectively as part of the practice team andpossess the skills and resilience to manage rapid change. Although prior experience in General Practicewould be an advantage, pharmacists from other sectors with significanttransferable skills will be considered for the role.

Person Specification

Qualifications

Essential

GPhC registered pharmacist and must hold a a GPhC independent prescribing qualification. Have a minimum of 2 years working as a pharmacist demonstrated within a practice portfolio. Experience in managing pharmacy services in primary care. Experience and an awareness of common acute and chronic conditions that are likely to be seen in general practice. Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email and face to face

Desirable

Minor ailments certification, membership of the Royal Pharmaceutical Society. Working towards faculty membership of the Royal Pharmaceutical Society Medicines Management Qualification MUR and repeat dispensing certification

Experience

Essential

Minimum of two years working as a pharmacist demonstrated within a practice portfolio An appreciation of the nature of GPs and general practice. An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing Experience in managing pharmacy services in primary care Experience and an awareness of common acute and chronic conditions that are likely to be seen in general practice

Desirable

In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare Understanding of the mentorship process An appreciation of the new NHS landscape including the relationships between individual practices, PCNs and the commissioners Broad knowledge of general practice
