Hounslow Mental Health Integrated Network Team (MINT) is a dynamic and integrated community and outpatient mental health team committed to providing comprehensive and high-quality mental and physical health care that addresses the needs of our diverse community. We accept referrals for service users transitioning from Children & Adolescents up to Specialist Older Adults. We are currently seeking to expand our multidisciplinary team and are inviting applications for the position of Mental Health Nurse. This opportunity will allow you to work alongside a dedicated team of professionals focused on improving the mental and physical health of our community under the Community Mental Health Framework for adults.
You will play a key role in dissolving the barriers that can exist between mental and physical health, social care, voluntary and community social enterprise (VCSE) and local communities. You will be working in secondary care, Monday to Friday, to deliver integrated, personalised, place-based and well-coordinated care for adults. We are moving towards a 'whole system' of mental health care, which will provide timely access to support, care and treatment.
Main duties of the job
As a community based Mental Health Nurse your duties will include but not be limited to:
Autonomously carry a caseload of routine to complex cases updating care plans and risk assessments as required
Screening patients for physical and mental health needs such as weight management, diabetes risks and smoking cessation and providing education to manage these.
Assist with management and administration of the local depot and physical health clinics
Medication management including monitoring of stock levels of the service to enable nursing staff to administer depots
Provide advice to service users and their triangle of care on the management of their mental and physical health, medications and any contraindications
Liaise with external stakeholders such as GPs, primary care networks, inpatient wards, crisis teams, social care and housing regarding ongoing needs
Assist with the transfer of cases between services or back to the GP at the end of treatment.
Escalate concerns to wider MDT using local processes
Identify and participate in local and service line service improvement using approved methodology

Professional qualification in mental health. Registered Nurse on the NMC register
Desirable
Educated to degree or Diploma level with other courses relevant to primary care mental health services CBT, Brief Solution Focussed Therapy etc RGN/physical health qualification Evidence of recent and on-going relevant further professional development
Experience

Demonstrable experience of post qualifying work in psychiatry. Experience of supervising junior staff within own and/or other disciplines
