Job overview

An exciting opportunity has arisen for a dedicated and motivated BACPR (Level 4) qualified Cardiac Rehabilitation Exercise Instructor to join the East Suffolk Cardiac Rehabilitation Team. 

You will take an active lead in the development of the exercise component of our established community Cardiac Rehabilitation Programme. You will assess, plan, and design a comprehensive Cardiac Rehabilitation Programme that is tailored to meet the individual needs of each patient, this may be home based or within a group setting. During each group, your will monitor, evaluate and record patient outcomes, offering advice and support.

You must have excellent verbal and non-verbal communication skills, be confident in working across professional boundaries, be flexible, innovative and a good team player with the ability to join our enthusiastic multi-disciplinary team.

Main duties of the job

To communicate effectively with patients and carers to maximise the patients understanding of their condition and their potential benefit from cardiac rehabilitation. Communication skills will need to include listening, empathy, motivation, persuasion and explanation to patients with a wide variety of needs. There may be language and other barriers to effective communication. To take on an educational lead role for exercise within the team and set up teaching sessions to team members and students or new starters. To work alongside the band 7 to improve service delivery focusing on quality of care to patients from an exercise perspective

To assist with the provision of an evidence-based approach to practice with the team that adheres to national and local guidelines for Cardiac Rehabilitation National Institute of Clinical Excellence (NICE), British Association for Cardiovascular Prevention and Rehabilitation (BACPR) and Association of Chartered Physiotherapists in Cardiovascular Rehabilitation (ACPICR)

·Monitor patient progression, adjusting patient programmes as required and ensuring that concerns are escalated to the clinical team.

Experience

Essential criteria

Proven experience of working in Cardiac Rehabilitation Experience delivering group exercise sessions Experience of meeting diverse needs of patients/clients Ability to work within a multidisciplinary team and to work well within a team environment Experience in assessing and planning holistic programmes of rehabilitation to meet the individual needs of the patient. Experience and knowledge of adaptations required for individual patients especially with co-morbidities

Desirable criteria

Mentoring / supervision of junior staff or students Audit and evaluation within cardiac rehabilitation programmes.

Knowledge

Essential criteria

Understanding of philosophy, of Cardiac Rehabilitation The pathophysiology, aetiology and epidemiology, diagnosis and management of coronary heart disease and its related co-morbidities. Exercise pre-screening, assessment, prescription, monitoring, supervision, and demonstration for individuals with CHD. IT literate and competent

Desirable criteria

Organisational infrastructure of the NHS and the health care system. NHS patient information systems
