Job summary

West Kent Primary Care are seeking to employ a Clinical Pharmacist on behalf of Tonbridge Primary Care Network (PCN).This is an exciting opportunity to join West Kent Primary Care as part of the established pharmacy team in Tonbridge PCN. The role is patient facing and will involve clinically assessing and treating patients using your expert knowledge of medicines for specific disease areas, provide expertise in medication reviews, support patients to manage long term conditions through medicines optimisation.

The post holder will work with our existing clinical pharmacy team at one of the Tonbridge member post holder will be supported to develop the role with the CPPE diploma and non-medical prescribing if they do not already hold these qualifications.

Tonbridge Primary Care Network (PCN)is made up of 5 member GP practices servicing over 50,000 patients - who offerservices ranging from First Contact Physio, Social Prescribers, and ClinicalPharmacy to make a range of services more accessible to our patients.

Main duties of the job

The Practice-based Clinical Pharmacist will improvethe quality and safety of care offered to patients of the Tonbridge. Theywill have regular contact with patients and will reduce the workload of GPprescribers in the practice. They will also support the management of patientswith long-term conditions.

Job responsibilities

Work as part of a multidisciplinary team to manage medicines on transfer of care and systems for safer prescribing. Perform face-to-face medication reviews of patients with polypharmacy, especially for those with frailty and/or with multiple co-morbidities and prescribe alternative medications. Undertake face-to-face reviews of patients with specific long-term conditions that fall within their competency (such as asthma reviews). Carry out Improved Access and Extended Hours sessions, including some weekend work. Provide leadership on quality improvement and clinical audit, as well as managing aspects of medicines safety and antibiotic stewardship. Contribute to practice financial stability through optimisation of medicines related targets in QOF delivery. Person Specification

Previous experience as a community / acute / primary care based pharmacist Experience in providing clinical advice and expertise on medicines to colleagues and patients
