Job summary

Hyper acute and acute stroke services across the Coventry and Warwickshire regions, designed to support and develop a changing stroke service within the region have been centralised. This post holder will be an integral part of this service; clinical pharmacy leadership; governance support and provide expert advice.

The primary role of this specialist post is to lead and provide a comprehensive, high quality pharmacy service for stroke patients with in the trust and ensure appropriate handover of repatriated patients across the region. In addition to working within a wider multidisciplinary team the post holder will lead the pharmacy service for the stroke speciality.

Main duties of the job

Lead, provide and develop a clinical pharmacy service for stroke patients at UHCW NHS Trust. Lead a pharmacy team to help operationally and clinically deliver pharmaceutical services to the stroke ward. Work closely with the pharmacy neurosciences team, along with other relevant surgical and medical pharmacists, supporting and covering, while developing services together. Work with other healthcare providers both in the community and at other hospital sites where UHCW NHS Trust is the regional hub for acute stroke services. Be responsible for all aspects of medicines optimisation including the cost-effective use of drugs and support of anticoagulation prescribing within stroke services, ensuring stroke patients receive safe, effective and timely drug therapy.

Job responsibilities
