Job summary

The Bristol Dementia Wellbeing Service is a joint venturebetween Alzheimers Society and Devon Partnership Trust. We provide Dementiaservices to people who are registered to a Bristol GP. We are located across two community hubs and support the people of Bristol with Dementia to live aswell as possible, for as long as possible, in their own community.

An exciting opportunity has arisen for a qualified professional to join us to support people with Dementia and their carers, families and staff.

The successful candidate will work closely with a designated group of GPs or Care Homes in a liaison model and hold a caseload of people who require Dementia assessment and diagnosis or support and treatment.

As part of a multidisciplinary team, you will attend a weekly team meeting where you will have access to support from peers and a range of professions and experienced staff.

This is an opportunity for applicants who will have transferable skills from previous experience working in Mental Health, Dementia or Physical Health and from a perspective which may be General Nursing, Mental Health Nursing, Physiotherapy, Occupational Therapy, Social Work or a variety of other professional backgrounds.

Main duties of the job

This role issuited to an experienced qualified clinician. Working inthe community using a range of core competencies (dementiafocused) to work with the GP and or Care Home to support the needs of peoplewho present with a range of memory loss, behaviour changes and changes incognitive functioning symptoms.

Aspart of the service, you will provide access to credible specialist dementia expertisefor GPs, Primary Care and Care Home staff and others who are requiring supportand expert advice on meeting the needs of those persons with a diagnosis ofdementia.

It willinclude case management (design and co-ordination of care/support that ispersonalised and evaluated). The liaison with a wide range of statutory andnon-statutory agencies is central to this role.

You will contribute to the clinical leadership of the team, providingsupport and informal supervision for other members and participating in teamand service development and evaluation.

You will have access to supervision and appraisal to develop your own area ofexpertise to support the service to develop and meet the needs of the localcommunity more effectively.

This rolewill be allocated to a designated hub but will be flexible to work across thecity of Bristol as required.

Communication and Working Relationship Skills

Contribute to the teams engagement and liaise with a wide range of stakeholders including: General Practitioners, Other Primary care staff, Services Users and Carers, Health and social care teams, Third sector/voluntary agencies, Community groups and local authority provision, Other statutory agencies

Provide specialist knowledge to support others to a greater understanding of dementia across the wider community

Provide education and training programmes re: dementia

Impart complex and sensitive information, which may be condition related understanding the perspective of others

Respond to requests to complete assessment, review and work with dementia crisis positively and expertly

Communicate information in a way that makes it relevant, sensitive and understandable

Provide and receive information some of which may be contentious, sensitive and in a highly emotive atmosphere to and from individuals and their families/carers

Provide high standard of written and verbal communication that is clear and in line with professional documentation standards

Participate in and chair clinical meetings

Facilitate access to social care and other health services

Promote social inclusion regardless of age, ethnicity, gender, sexual orientation or disability

Use IT effectively and appropriately

At all times ensure the person with dementia, their carers where appropriate, are included in any communications about them.

Person Specification

Qualifications

Essential

A professional mental health qualification and registration eg RMN, Social Work Degree or equivalent, BSc/Diploma in OT or Community Nursing Degree.

Desirable

Non Medical Prescriber and/or another recognised therapy qualification Post graduate qualification relevant to specialism Clinical Supervision qualification Training to level 3 in Safeguarding Adults and children and young people: roles and competencies for Health Care staff

Experience

Essential

An understanding of the Mental Health Act/ Mental Health Capacity Act requirements. Understanding of community structures and Health and Social Care Agenda for Older Adults Understanding of the Ageing Well principles in Dementia care Understanding of the organic presentations in Older People

Desirable

An understanding of the CPA Process

Skills & Abilities

Essential

Substantial experience of managing clinical complexity Working within a community setting in either the statutory or voluntary sector. Experience of lone working and able to make clinical decisions independently

Desirable

Providing clinical supervision. Experience of working in a multi-disciplinary and multi-agency environment. Participation in user involvement in services. Experience of delivering model based psychological therapy

Communication and Working Relationship Skills

Essential

Excellent communication skills including the ability to engage with people experiencing a wide range of needs in relation to dementia. Ability to communicate with a wide range of professionals, community services etc. from various settings. Able to produce well written reports.
