The Pre-assessment nurse will work as part of the pre-assessment team in order to assess the fitness of all elective surgical patients for their planned surgical procedure. All specialities will be assessed including General surgery, Orthopaedics, Urology, Gynaecology, ENT, Max Fax, Plastics, Breast surgery, Dental and Ophthalmology. The pre-assessment nurse will also provide patients with up to date verbal and written information in order to inform consent. The pre-assessment nurse will be supported by the pre-assessment lead nurses and the Consultant anaesthetists. This is a highly skilled role in which you will learn to optimize a patient's fitness to undergo anaesthesia and surgery. A wide knowledge of clinical conditions and co-morbidities is needed in order safely assess patients. Staff work autonomously and independently using clinical judgement to make the correct decision on any action to be taken to optimize the patient's health and social care.

Main duties of the job
The post holder will review the patients' Pre-assessment health questionnaire with the patient, in order to highlight any potential problems for undergoing a General anaesthetic.
The post holder will prepare elective surgical patients for procedures and order appropriate clinical investigations according to agreed protocols to aid thorough assessment of the patient prior to admission.
The post holder will work closely with the extended multi-disciplinary team to optimise patients fitness and preparation for surgery pharmacist, anaesthetist, complex needs team etc. Organising and participating in the delivery of high standards of evidence-based nursing care.
Taking responsibility to carry out required planned, direct patient care to a high standard without direct supervision, in accordance with established nursing procedures and policies.
Contributing towards the health promotion for all patients by provided education, support and advice as necessary, ensuring referral to appropriate professionals or agencies as required.
In the absence of senior staff, taking responsibility for the delegation and supervision of appropriate work to other qualified staff, healthcare support workers and students.
Where relevant, be responsible and assist in the co-ordination of discharge planning in partnership with the multidisciplinary team.

Job description
Job responsibilities
Work to agreed protocols and guidelines bothnational and local to carry out high standard pre-operative assessments onpatients.
Assist in the planning of pre assessment clinicsand carrying out pre-operative assessments to ensure patient is fit for surgeryfor day case and inpatient procedures and minor and major surgeries.
Work under the indirect supervision of a seniornurse
Provide clinical advice and support to staff andpatients within own level of competence
Access, plan implement and evaluate holistichigh quality and individualised care which respects a patients dignity andbeliefs.

Perform and organise investigations includingvenepuncture, physical examination, swabs, BP, ECG, spirometry, echocardiogram,sleep studies and referrals to complex needs teams where appropriate to ensurethe patient is optimised for surgery.
Interpret results and act on them accordinglydocumenting any action taken and referring to Anaesthetic opinion whererequired and liaising with MDT involved in patients pathway.

Responsible for own and whole departmentsrequirement to reduce hospital acquired infection and cross infection throughthe implementation of good practice effective hand washing; daily dampdusting of all equipment; using protective clothing appropriately; rigorouslyapplying standard (universal) precautions to minimise the transmission ofinfection.
To communicate individual patients specific careneeds to the clinical and clerical support worker team so as to ensure allrelevant information is shared to the waiting list.
To escalate any concerns regarding suitabilityof a patient to undergo surgery with the consultant in charge of their care andthe consultant anaesthetist.
To provide health education and optimisation topatients including smoking advice, alcohol advice and weight and diet.
Be willing to undertake role specific trainingand attend study days/courses to develop self and teach others.
