This an exciting opportunity to join the neonatal outreach service. 

You will be expected to provide a transition from hospital to home for those babies requiring some follow up prior to discharge to the health visiting teams. 

Supporting families at home and providing advice via phone and in their own homes. 

This position will cover discharges from Worthing hospital, with some cross site annual leave and sickness cover with St Richards, Chichester and will require the post holder to hold a driving licence and have access to their own car for work purposes. 

Main duties of the job

The outreach nurse will use their knowledge and experience to provide a seamless transition from hospital to home, supporting parents and families to meet the ongoing care needs of their infants. The post holder will provide safe ongoing management of medical needs until appropriate time for discharge from outreach. Promote progressive attitude to continuous improvement of patient care at home through evidence based practice. To work some clinical shifts within the neonatal services as part of the wider nursing team if required.

Detailed job description and main responsibilities

Work in partnership with the neonatal team, families and community services. Work in conjunction with the multidisciplinary team, to promote timely discharge. . Write precise, accurate and contemporaneous nursing records, reports, statements and other correspondence. Document all social interaction and communication between parents and the multidisciplinary team, enabling the team to plan appropriate care, such as the baby at risk. . Communicate sensitively, with families listening actively, offering support and informed advice. Promote health related behaviours to families/carers.

Registered Nurse or Midwife Mentorship module or equivalent Current UK driving licence and own car Neonatal QIS
