PCN Paramedic

Job summary

To work aspart of an MDT within the PCN to provide patient-focused care responsive tohealth need demonstrating strong communication at all levels across theorganization.

To providefirst point of contact direct access for patients of all ages withundifferentiated and undiagnosed problems seeking a primary health care serviceincluding on-call, telephone triage, face to face consultations, and homevisits.

To assess, triage and manage patients within a variety of settings including homevisits with undifferentiatedandundiagnosed health careneeds including investigating, treating,and referring patients as appropriate.

To order investigations, undertake and follow-up resultsand arrange appropriate management including seekingadvice where appropriate.

Provide adviceto patients on general healthcare and promote self-management whereappropriate, including signposting patients to the PCNs social prescribingservice and other services where appropriate.

To work with a multidisciplinary team including community and voluntarysector to improve patientcare.

Actively take a personalised care approach and population-centred care approach to enable shareddecision making with the presenting person.

Maintainaccurate and contemporaneous health records within SystmOne appropriate to theconsultation, ensuring accurate completion of all necessary documentationassociated with patient healthcare.

Main duties of the job

Thesuccessful candidate will have an Undergraduate Degree Qualification in;Paramedic Science, Clinical Practice, or an equivalent qualification, withevidence of academic study and practical experience of minor/major illnessesand injuries that are typically encountered in a primary care setting.

Has threeyears experience as a paramedic at band 6 (or equivalent).

You willneed to be registered with HCPC. It is a requirement for the candidate to havea desire to gain a post graduate qualification in Prescribing, if not alreadyheld.

Achieved orwilling to work towards Masters level (level 7 equivalent) in paramedic areasof practice.
