An exciting opportunity has arisen for a Band 5 or 6Occupational Therapist to join our Therapy Team at Petersfield CommunityHospital. The post holder will be providing advanced clinical and professionalOccupational Therapy/Physiotherapy to the Therapy Team or Community HospitalTeam, and to provide support to the team leaders.

Undertake assessments of patients with diverseand complex conditions, providing a clinical diagnosis, development, anddelivery of individualised treatment programmes.
To support the case management process byproviding expert clinical advice and therapy to patients with a variety ofcomplex conditions
To be skilled in assessing and treating a rangeof complex factors relating to the patients condition, diagnosis and prognosis
Negotiate and agree with the patient, carers andother care professionals, individual roles and responsibilities with actions tobe taken and outcomes to be achieved, referring on to other services orprofessionals as appropriate.
Undertake comprehensive assessment of patientsincluding those with diverse and highly complex presentations and multiplepathologies, using clinical and analytical skills to provide an accurateassessment of their condition

Shift Pattern: 8:30-16:30

Enhanced DBS Degree level qualification or equivalent To be registered with the Health and CareProfessions Council (HCPC) Experience of working as part of amultidisciplinary team NHS experience Good communication skillsPerson Specification
