Job summary

We have a great opportunity to join our dietetic team at Ashford and St Peter's Hospital. We have a vacancy for a 12 month fixed-term contract , Part Time B6 Community Dietitian. We are looking for an enthusiastic and confident Dietitian who can work both autonomously and as part of our Community Dietetic team based at Ashford and St Peters Hospital. We offer support to a range of patients in care homes and the community requiring nutrition support advice, enteral feeding, and those with complex needs who are unable to attend outpatient clinics. The team also provide support to inpatients at Walton and Woking community hospitals, as well as education and training to meet care home needs. Team members work closely with SLT colleagues, other health care professionals, palliative care teams, carers and patients and family members to ensure that patients are receiving optimum levels of appropriate nutrition and hydration, provided in a safe and supported environment.

Main duties of the job

The Community Dietetic team form part of the Ashford and St Peter's Dietetic Department to provide nutrition and dietetic services to CSH community-based patients in need of dietetic advice and review. This includes establishing and monitoring home based patients on enteral feeds.

As a practitioner with a high degree of autonomy you will take responsibility for planning and delivering your own caseload acting as a source of expertise on dietetic/nutritional management to patients, carers, GPs, consultants as well as other health care professionals in both hospital and community settings.

Flexibility, and good organisational and prioritisation skills are essential to this community-based role. The successful candidate should be able to demonstrate excellent team working skills and enthusiasm. The post holder must have a valid UK licence and access to a car. They will be responsible for organising and providing nutritional care plans and services for patients, carers, and staff.

The post-holder will be play an active role within the general functioning of the Dietetic Department, including contributing to student training, audits and service improvement projects.

To post holder will communicate and collaborate with a variety of team members, other HCPs and partner organisations to support service improvements and deliver high quality patient care. They will participate in the teaching and training of other health care professionals.

Job responsibilities

The post holder will act as a specialist for the local community supporting the delivery of the dietetic service provision to community Hospitals in-patients and staff and to community-based patients in need of dietetic advice and review.

The post holder will undertake comprehensive assessments on nutritional management of patients on Home Enteral Nutrition, supporting their carers as well as other hospital and community health care professionals. They will be involved in developing and delivery of specialist nutrition and dietetic education and training to care homes.
