Older Peoples Medicine is one of the largest specialities in the hospital and is led by a motivated, forward-thinking team. We offer a range of services across our five dedicated Older People's Medicine wards including a short-stay ward, an ortho-medical ward, a ward specialising in dementia care, a complex care ward and the UK's first Older Peoples Emergency Department. We have a dedicated dementia support team, integrated specialist volunteers and a programme of stimulating activities and events helping to provide the best experience for our patients in hospital.

As a tACP, you will begin a structured pathway of education and training to develop your skills to further your career and develop the required skills, knowledge and behaviours needed to practice as an Advanced Clinical Practitioner in Older People's Medicine.

Main duties of the job

The ideal candidate will have significant knowledge and experience gained within Older Peoples Medicine; you will be a team player with an enthusiastic and flexible approach to working. You should have excellent communication skills and be committed to providing the best possible emergency care to Older People in the East of England.

Our teams enjoy the diagnostic challenge of working with older patients with complex needs and the benefit of sharing skills with clinical colleagues from other specialties. Within OPED you'll have the opportunity to develop a broad understanding of the needs of older people, build on your specialist knowledge and experience within emergency and older peoples medicine and take advantage of the opportunity to progress as a clinician in this ever-changing area.

Our team currently consists of seven ACP's from a variety of professions including nursing, paramedic, physiotherapy and pharmacist backgrounds. We work autonomously across all areas of the Emergency Department and have a strong working relationship with the remainder of the Emergency Department team, community team and pharmacy department. You will provide high quality, patient focused, Comprehensive Geriatric Assessments of complex patients lead by a Consultant Geriatrician.

Job responsibilities

The post holder will be expected to exercise a high degree of personal professional autonomy and make critical judgements to satisfy the expectations and demands of the role and service. Assimilate highly complex strands of information autonomously, to reach a differential diagnosis, order appropriate investigations and begin first line treatments in line with local and national guidelines. Demonstrate comprehensive, efficient, timely and targeted clinical history taking. Where appropriate is able to get a collateral history from carers which includes pre-admission level of function, cognition and changes and specifics of any care packages in place with an understanding of the information needed by other members of the MDT and why that information is needed to ensure questions are asked and documented at the right level of detail. Demonstrate ability to perform both a comprehensive physical assessment of all body systems and a mental health assessment and demonstrates a more focused clinical examination directed at the presented complaint, actively seeking and eliciting relevant positive and negative signs and subtle findings to assist their diagnostic process. Demonstrates the ability to interpret atypical presentations of illness of older people and detecting treatable acute conditions which may not be immediately obvious. In a complex, non- emergency situation and together with the MDT demonstrates the ability to: develop a provisional diagnosis and differential diagnosis on the basis of clinical evidence which takes into consideration physiological changes of ageing; institute an appropriate investigative and therapeutic plan which takes into account the patients and carers wishes, their cultural and spiritual priorities, and their capacity and best interest decision making.

Essential

Professional Registration with either NMC, GPhC or HCPC Possession of degree in a clinical field , Nursing, Physiotherapy at a minimum 2:2 classification. Or evidence of level 6/7 module undertaken. MSc in Advanced Practice or equivalent level of experience ( registered on Centre of advancing practice directory) supplemented by specialist training & competency sign off by Consultant in field and other specialist qualifications relevant to the post, including MSC (level 7) modules / qualification.
