Job Purpose:

To work within the Childrens Community NursingService, delivering high quality, evidence based nursing care for children andyoung people with short and long term conditions.

To identify the health needs of children to benursed in the community and work in liaison with multi-agency partners topromote collaboration and effective communication.

To work across the Childrens Nursing Servicedeveloping skills in respiratory; epilepsy; complex and acute care.

Duties and Responsibilities:

Post holder to work flexibly across the Childrens CommunityNursing service, delivering high quality, evidence-based nursing care forchildren and young people with short- and long-term conditions including supportingchildren with life limiting and life threatening conditions, preventinghospital admission and facilitating early discharge.

Postholder to work within the Childrens Community Nursing Service, delivering highquality, evidence-based nursing care for children and young people with short-and long-term conditions including supporting children who attend hospital forsurgery and facilitating early discharge.

Toidentify the health needs of children to be nursed in the community and work inliaison with multi-agency partners to promote collaboration and effectivecommunication.

Tofacilitate early discharge from the hospital/A & E by providing care athome as an alternative to hospital admission.To act as a specialist resource developing strong links with GeneralPractitioners, Health Visitors and School Nurses.

Toassist in the ongoing development and management of the Childrens CommunityNursing Service.

Toaccept responsibility for own professional practice as a reflective,accountable clinical practitioner.

Clinical Management and CareDelivery

Receive referrals into the service and ensure robustassessment, planning, implementation, and evaluation of programmes of care thatare family centred and foster the principles of clinical excellence.

Deliver high quality nursing care, health promotion andeducation that contributes to well-being and illness prevention, by ensuringboth clinical practice and all verbal and written information is evidencebased.

Ensure care at the point of delivery is in accordance withthe Trust agreed clinical standards, policies and guidelines and that care isgiven in a competent manner that recognises own limitations and seeksappropriate supervision as necessary.

Possess up to date clinical skills to closely monitor thechilds progress and improvement, and to detect deterioration promptlyfollowing the agreed care pathway to medical reassessment.

Provide accurate updates to senior staff of the childsphysical and, emotional wellbeing and on the progress of their condition /diagnosis. Report promptly any deterioration in their condition.

Undertake clinical tasks, which require advanced skills, administration of IV antibiotics, respecting the rights of children andyoung people and ensure appropriate consent is obtained prior to carrying outany clinical procedures.

Actively participate in the supervision and support ofjunior staff, delegating and prioritising duties, tasks and workload.

Contribute to efficient budget management, by optimising useof resources, reporting any anticipated budgetary needs or pressures.

Participate in the discharge process, both to and from theservice, ensuring appropriate referral onto other healthcare staff or agenciesif care dictates.

Be able to provide accurate up to date activity data relatedto service provision.

The post holder shall fully participate and were necessarylead on the Common Assessment Framework, to ensure that all children withadditional needs are provided with a co-ordinated multi-agency response tomeeting their needs.

To work flexibly to cover the needs and specification of the6 day a week service, including weekends and out of hours provision.

Communication and OrganisationalAwareness

Promote good communication links between primary, secondaryand tertiary services, endeavouring to foster the principles of seamless care.

Attend and participate in multi-disciplinary meetings,safeguarding procedures and case conferences as required.

Be aware and participate in risk assessment for the benefitof self, colleagues and clients in line with current trust policies, withparticular emphasis on the Lone Worker Policy.

Ensure that verbal communications and written documentationare maintained and are clear, relevant and concise, in accordance with Nursingand Midwifery Councils (NMC) Standards of Records and Record Keeping andcurrent Trust policy.

Maintain confidentiality of information and ensure safecustody of all records, in accordance with Trust policies and NMC guidelines.

Actively promote the services philosophy and values tosupport the child and family, whilst always acting as an advocate for the childand family.

Be responsible for reporting any accidents, incidents orcomplaints as per Trust policy.

Education and Training

Provide advice and training on the management and treatmentof paediatric conditions. Supportchildren and families according to their level of knowledge, ability andwillingness to participate in care delivery.

Participate in the development of staff within the service,promoting professional development within the service in order to developskills and competencies, in accordance with the Knowledge and Skills Framework(KSF).

Assist and support training and assessment of junior staff,childrens support workers and student nurses, ensuring their learning outcomeslink theory to clinical practice.Participate in the induction programmes of newly appointed staff; attendTrust mandatory training sessions as required.

Participate in the delivery of teaching sessions as part ofthe day-to-day delivery of the service.Within the service, promote teamwork, share new skills, knowledge andexpertise, which is proven best practice and evidence based.

Leadership and Responsibilities

To raise local awareness across primary, secondary andtertiary care, as to the functions, capacities and work patterns of theChildrens Community Nursing Service.

To participate in the formulation of guidelines, protocolsand policies that reflect the needs of the target care group.

Initiate evaluation of current nursing practices and becomeinvolved with audit and clinical governance, which will inform new ways ofworking.

Participate within staff performance reviews for ChildrensCommunity support workers and the formation of personal and service developmentplan. Provide clinical supervision forjunior team members.

To adhere to all Trust policies relating to the storage andadministration of all medications, promptly reporting any discrepancies to therelevant line manager

Embrace the principles of Child Protection working withinthe Framework for Assessment and Safeguarding Children guidelines.

To be responsible for managing own daily workload andplanning, prioritising and delivering care effectively, whilst identifying andmanaging any risks to the quality of care provided.

Professional and EthicalResponsibilities

To adhere to professional standards in accordance with theNursing and Midwifery Councils (NMC) Standards of Professional practice and Codeof Professional Conduct.

Maintain own professional development andportfolio/appraisal systems, identifying personal clinical supervision needs inline with PREP and registration requirements.
