Main duties of the job

The post holder will demonstrate critical thinking,be a caring, compassionate, andcommitted nurse who, acting within theirprofessional boundaries, will provide care for the presenting patient chronicdisease management such as diabetes, coronary heart disease annual healthchecks, health promotion as well as cytology, childhood immunisation, travelvaccines, health checks and dressings. They will demonstrate safe, competentclinical decision-making and expert care, including health promotion for patients.

The post holder will be supported to become a nurseprescriber.

About us

We have Investors in People Silver Award and are committed to developing our team to provide quality care to our 10,000 patients. We are a training practice and teach GP Registrars. We are a member of Greater Middlesbrough PCN and work closely with our other 7 member practices.

Our Primary Care multidisciplinary team currently consists of 4 GP partners, 3 salaried GPs, 2 practice nurses, 2 health care assistants, clinical pharmacist, and pharmacy technician. We also have allocated ARRS roles via our PCN which included FCP, Dietician, social prescribers, mental health work, among others. Attached staff include midwife, health visitors and district nursing team.

Job description

Job responsibilities

Chronic Disease Management, diabetes, coronary heart disease, COPD, asthma, learning disabilities, severe mental health and agree management plans. Annual health checks working to Ardens Templates. Proactively offer health promotion. Dressings, injections, vaccinations and immunisations, cytology screening and injections flu, pneumonia, shingles, RSV. Annual health reviews to national and practice protocols, and within scope of practice Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care Support and promote national screening bowel, breast, cytology. Promote and deliver evidence-based care for patients requiring ear syringing.

Delivering a quality service

Recognise and work within own competence and professional code of conduct as regulated by the NMC Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies, and procedures Deliver care according to NSF, NICE guidelines, and evidence-based care Assess effectiveness of care delivery through self and peer review, benchmarking, and formal evaluation Initiate and participate in the maintenance of quality governance systems and processes across the practice Use the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required In partnership with the primary care team, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate by attending internal quality improvement and clinical meetings Support and participate in shared learning across the practice Use a structured framework ( root-cause analysis, reflection) to manage, review and identify learning from patient complaints, clinical incidents, and near-miss events Contribute to the appraisal of the performance of HCAs, providing feedback as appropriate Understand and apply legal issues that support the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patients health procedures and local guidance Working with practice management to ensure the whole team have skills and knowledge regarding domestic violence and vulnerable adults. Provide guidance and support to ensure appropriate referral if required

Management of risk

Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients Monitor work areas and practices to ensure they are safe and free from hazards and conform to health, safety and security legislation, policies, procedures, and guidelines Ensure appropriate supervision of safe storage, rotation and disposal of vaccines and drugs. Oversee the monitoring, stock control and documentation of controlled drug usage according to legal requirements where appropriate Working with the practice management, ensure the nursing team has access to and undertakes mandatory and statutory training requirements Apply infection-control measures within the practice according to local and national guidelines Advocate for policies that reduce environmental health risks, are culturally sensitive and increase access to health care for all Interpret national strategies and policies into local implementation strategies that are aligned to the values and culture of general practice

Managing information

Use technology and appropriate software as an aid to management in planning, implementation, and monitoring of care, presenting and communicating information Review and process data using accurate SNOMED codes to ensure easy and accurate information retrieval for monitoring and audit processes Understand responsibility of self and others to the practice regarding the Freedom of Information Act Undertake mentorship for HCAs, assessing competence against set standards Disseminate learning and information gained to other team members to share good practice and inform others about current and future developments ( courses and conferences) Assess own learning needs and undertake learning as appropriate Person Specification

Experience

Essential

An understanding, acceptance and adherence to the need for strict confidentiality. Ability to use own judgment, resourcefulness and common sense. A commitment to maintain a high professional standard of nursing care and keep up to date with all aspects of nursing care relevant to the post. A commitment to ensure all Health and Safety requirements and Infection Control measures are met and to report any problems to management. A commitment to the effective use of Practice and NHS resources An awareness of own limitations and experience. To work only in accordance with the UKCC Code of Conduct and within the Scope of Professional Practice. Chronic Disease management skills, management of patients with long-term conditions. Dressings, venepuncture, basic observations, cytology screening, injections, vaccinations, and immunisations. Ability to work to PGDs and NMC code of practice. Communication skills, both written and verbal.
