Provide specialist support and advice relating to the development of secondary care specialist services for people with diabetes.
In Conjunction with Diabetes nurse specialists provide support to develop services across BCUHB.
Be an integral member of the multidisciplinary diabetes team which provides support to general practice in the direct care of patients and within locality based community clinics and be responsible for ensuring that the services are integrated into existing services within the community and secondary care.
Provide advice, support and training to all secondary, primary and community healthcare professionals.
Have the freedom to act, influencing patient care and contributing to service provision.
Provided specialist advice and support and will participate in the education and development of staff working with patients with diabetes.
Work collaboratively and co-operatively with colleagues to develop services and quality of care delivered both within secondary, primary and community areas.
Main duties of the jobSupport the management of patients offering expert advice with regard to the patient with diabetes and their carers alongside other health professionals.
To provide specialist diabetes education and support to people within secondary community and primary care.
Provide health promotion and expert clinical knowledge to patients, carers and relatives.
Through education and using expert clinical skill and knowledge will actively involve patients, carers and relatives in the planning, delivery and evaluation of care.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
