Working with people living with life limiting illness whether that be in their homes or in the Hospice is very different to working in a hospital. You have more time to spend caring for each patient, getting to know them and their support network and can make a real difference in helping improve their quality of life.

We are looking for caring, empathetic and compassionate registered nurses with excellent communication skills, who are organised and able to assess, prioritise and understand the needs of those receiving palliative end of life care.

Working as part of a team, our palliative care nurses use a patient centred approach and evidenced based practices to administer medication and treatment as prescribed in accordance with the NMC and the Trusts policies.

Main duties of the job

Be responsible for assessing, planning, implementing and evaluating programmes of care, actively seeking evidence-based practice which will improve the quality of care and meeting the needs of the In Patient Unit (IPU).

Ensure the highest possible standards of care are received by patients and those important to them, adhering to statutory regulations and Hospice policies.

Be an active member of the multi-professional team and treat all patients with respect and courtesy, maintaining dignity and respecting individual needs and differences, both physical and cultural.

Lead shifts on the IPU when required, ensuring appropriate allocation of workload as appropriate

Be responsible for the assessment, planning, implementation and evaluation of the person centred care for patients, including discharge planning

Our friendly, professional and nurturing team of approximately 150 employees and over 400 volunteers work side-by-side to ensure the Hospice provides specialist hospice care for adults towards and at the end of life.

Job responsibilities

Primary duties and key responsibilities

Ensure that all clinical assessments are completed on first assessment and reviewedregularly and especially when the patients condition changes.

Identify when individual risk assessments are required and liaise with an appropriatelytrained member of staff to ensure that these are completed.

Involve patients in all aspects of their care as much as they are able and acting as a patientadvocate when required, ensuring that those important to the patient are involved in theircare in accordance with the patients wishes.

Provide symptomatic relief if required by administering prescribed medications.

Develop and maintain effective communications with patients and those important to them,in a way that respects their views, autonomy and culture and contribute to emotionalsupport during admission, at time of death and bereavement.

Ensure that a suitable and safe environment is provided, whether on the ward or in thehome environment.

Lead shifts on the IPU when required, ensuring appropriate allocation of workload for theIPU and Community team when required.

Supervise the clinical practice of Healthcare Assistants and participate in the orientation ofnew staff including Bank or Agency staff.

Act as a mentor for work placements and visiting trainees.

Maintain nursing records according to mandatory legal requirements, ensuringconfidentiality is maintained at all times in accordance with NMC Guidelines Standards for records and record keeping.

Complete pharmacy requisitions as per policy and Standard Operating Procedures.

Liaise with other healthcare partners such as Community Nursing teams, ContinuingHealthcare, GPs, Community Physiotherapists and Occupational Therapists.

Prioritise workload according to changing clinical need for shift.

Contribute to setting goals and benchmarking standards of care. Develop tools to improvethe quality of the service and co-ordinate clinical services across the ward.

Record and report all incidents/complaints involving staff, patients and visitors inaccordance with Trust policies and assist in any investigations as required.

Work to ensure that clinical areas are well maintained and comply with health and safetyregulations, ensuring the safe use of equipment and storage of materials.

Comply with the NMC Professional Code of Practice and Behaviour for Nurses and Midwives.

Person Specification

Qualifications

Essential

Degree in Nursing. Registered Nurse with current NMC registration and evidence of recent CPD.

Desirable

Evidence of study in relevant subjects at level 6. Clinical supervision and/or mentorship qualification and/or a recognised teaching qualification.

Experience

Essential

Experience of working within a nursing team. Experience of dealing with vulnerable patients.

Desirable

Experience in community nursing, palliative care, end of life care, oncology or general medical nursing. Experience in research audit, standard setting and/or clinical governance. Experience in delivering training. Experience in the management of complex needs and medicines management at the end of life.
