Job summary
The right candidate will be capable of working independently on all practice matters and take responsibility for general practice issues on a day-to-day basis. The candidate will be able to provide excellent care to all our patients, work independently and manage own workload. Good communication and organisational skills are imperative to be able to fulfil this job role.
The candidate must be a RGN with current NMC registration with excellent clinical skills; Highly developed interpersonal and communication skills; Practice autonomously and demonstrate evidenced based clinical decision making.
The candidate must be trained and fully competent in a Chronic Disease Management (qualification in COPD or Asthma preferable), Cervical Screening, Childhood Immunisations, Wound management and Travel Vaccinations OR a willingness to work towards this.

Main duties of the job
To be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice. Experience in COPD/Asthma is preferable.
Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.
The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, chronic disease management, health promotion, as well as actively supporting the Lead Nurse in the reviewing and delivery of clinical policy and procedure.

We are a rural GP Practice running over two sites (a mile apart) with 7 GP Partners,5 salaried GP's, a-strong Nurse and HCA team and a large administrative team. Our patient list size is over 21,000 and growing due to new housing developments. We are a large, busy and dedicated practice who support the training and development of our staff in all a practice our main duty to the patient is not only to treat illness but promote good health and wellbeing. We endeavour to treat all our patients as individuals and equals.
Practice Facilities
The Practice is situated over 2 sites, Rothwell Medical Centre and Desborough Surgery.Rothwell Medical Centre is located on the outskirts of the town and benefits from car parking on site. Desborough Surgery is located in the centre of Desborough close to the library and other amenities. There is parking to the side of the surgery and nearby. Both sites are purpose built medical centers, have wheelchair access toilets and baby changing facilities.

CLINICAL RESPONSIBILITIES
Deliver high quality, compassionate care with a focus on promoting health, self-care, and independence.
Carry out specific delegated clinical tasks and responsibilities to a competent standard
Working without direct supervision (where appropriate) within designated parameters of practice of the Practice Nurse role, accessing clinical and care advice when needed
Recognise issues relating to safeguarding vulnerable children and adults and report any problems or raise concerns to the appropriate registered care
Exercise professional accountability and comply with practice policy in relation to the safe administration of medicines
Use electronic records to record the outcome of assessment and evaluation of care
Assist in the assessment of and management of risk across the environment where care is being administered
Assist with the implementation and monitoring of clinical standards and outcomes
Develop a working knowledge of other providers resources and referral systems to ensure individuals needs are met, within parameters of practice
Demonstrate good understanding of principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures
Demonstrate a good understanding of the Mental Capacity act / Deprivation of Liberties and applies principles to everyday practice seeking advice / guidance from Registered Nurse or another registered professional as required
Deliver effective care following plans determined by the registered nurse or other registered care professional and provide feedback on progress against the plan
ADMINISTRATION
Contributes and participates in audits, evaluation, and clinical standard setting within the Practice
Accurate and timely summarising of patient records and read-coding patient data
Complete all required paperwork for legal and administrative purposes in accordance with relevant standards
Ensure that all practice policies are fully implemented
Work in accordance with all governance and internal systems relating to the management of clinical data and systems

* Understanding of the scope of the role of the Practice Nurse in context of the nursing and interdisciplinary team and the organisation, and how the role may contribute to service development * Understands and acts in line with NMC professional standards for practice contained within The Code. * Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice * Understand requirements for NMC professional Revalidation Understanding of the importance of following procedures and treatment plans * Experience of working in teams under appropriate supervision as part of a multi-disciplinary team * Insight into how to evaluate own strengths and development needs, seeking advice where appropriate * Experience of providing and receiving complex, sensitive information * Ability to take part in reflective practice and clinical supervision activities * Ability to organise and prioritise own delegated workload
