Job summary

We are pleased to advertise the role of Digital and Transformation Lead within Health Village Dearne Valley PCN - a Primary Care Network collaboration of 3 practices based within the Rotherham area serving a patient population of 48,000.

The post holder is to support all aspects of the developmentand function of the Primary Care Network (PCN). They will work alongside theClinical Director, PCN lead and PCN Board members to build on current primarycare provision, implement improvement and transformation aligned to local andnational priorities and be committed to the shared purpose and vision of thenetwork.

Main duties of the job

The Digital and Transformation Lead will help to develop,guide and oversee the strategic aims and objectives of the Network, ensuringthat Practices and stakeholders have the relevant information and options tomake informed decisions. The post holder is able to implement agreed plans andprojects into business operations, with the ability to plan, monitor andevaluate practical improvements that will make a positive difference topatients and Practices within the PCN.

Working closely with a wide range of partners andstakeholders, both within the PCN and externally, the post holder will activelyseek out opportunities and projects in accordance with contractual requirementsand aligned with the PCNs strategy that will enhance patient care, addresshealth inequalities, and support the Practices with the PCN.

About us

Health Village Dearne Valley PCN is a collaboration of 3 practices based across Rotherham. We are an effective and efficient PCN with a successful track record of providing safe, effective and holisitic patient care.

You will work closely with the various Practice Managers across the 3 constituent Practices as well as alongside our Clinical Director. You will also work with, and oversee, a varied team of other PCN staff including Paramedics, Pharmacists and Pharmacy Technicians, Social Prescriber, Nurse Associate and Care Coordinators.

As a Digital and Transformation Lead within the Health Village and Dearne Valley PCN leading on the delivery of excellent patient care and access whilst protecting GP practices and the workforce.

To provide inspiration, expertise, and leadership to deliver changes and projects in a collaborative way. To allocate resources effectively within the PCN and contribute to the efficient resource management of the PCN To lead and manage service development and planning. To prepare, chair and influence strategic steering groups, and multi-disciplinary meetings that support the Networks vision. To consider the PCNs population health needs and health inequalities and provide a robust infrastructure to address the needs. To work build and maintain good professional relationships both within the Network and externally. To performance manage PCN employees and monitor quality assurance. To be committed to ongoing professional development To work proactively within the team and other teams to ensure integrated multidisciplinary working is successful To develop and oversee financial budgets and present to stakeholders on a regular basis. Evaluate budget proposals and financial requests for viability. To create and maintain effective open and constructive relationships To work closely with the Networks Clinical Director and PCN Leads to operationally implement the Networks strategic objectives. To undertake any other duties in agreement with the post holder and manager. Person Specification

Experience

Essential

Experience of working within a management role, ideally within primary care or a similar environment. Ability to provide inspiration, guidance and leadership to a range of stakeholders. Experience of managing complex funding streams and high-level contracts and the ability to interpret financial and contractual information for a range of audience. Experience of using programme management, methodologies and improvement techniques to drive change forward. Ability to utilise entrepreneurial skills to actively seek out opportunities that will positively impact the Networks Practices and patients. Ability to effectively resource manage Network employees and understand the implications within the wider Network. Ability to effectively negotiate and influence decision making using sound logic and researched evidence. Ability to work autonomously and manage own workload whilst managing a range of conflicting deadlines and priorities. Ability to work effectively and build and maintain good working relationships with a range of internal and external clinicians, managers, stakeholders and partners. The ability to identify risk and project management concerns early and take the appropriate actions. Experience of contributing and chairing multi-disciplinary meetings and encouraging multi-disciplinary working. Experience of gathering, providing and using information to support the Networks strategic and operational objectives. Ability to make informed and sound decisions independently quickly, whilst managing risk. Ability to communicate effectively to a wide range of audiences, both written and verbal. Ability to demonstrate personal professional resilience. Ability to travel to various locations within the locality and beyond. An Enhanced DBS check is required for the role.
