Job summary
The post holderwill demonstrate courageous and critical thinking and is a caring,compassionate and committed experienced general practitioner who, acting withintheir professional boundaries, will provide care for the presenting patientfrom initial history taking, clinical assessment, diagnosis, treatment andevaluation of care. They will demonstrate safe, competent clinicaldecision-making and expert care, including assessment and diagnostic skills,for patients within the general practice. The post holder will commit todemonstrating critical thinking in the safe clinical decision-making process.They will communicate and work collaboratively with the general practice teamto meet the needs of patients.
Main duties of the job
Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team Implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis Clinically examine and assess patient needs including history taking, assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs and onward refer where appropriate. Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice Assess, identify, and refer patients presenting with mental health needs in accordance with the NSF for Mental Health Implement and participate in vaccination and immunisation programmes for both adults and children Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person

MDCPCN comprises of 2 Practices in Mottingham, Downham and Chislehurst in LondonBorough of Bromley. The practices have a combined geographic area made up ofaround 35,000 patients. There is a wide socio-economic and demographic servedwithin this area which will provide an interesting and varied case mix. Youwill be joining an enthusiastic team of clinicians and administrators. The teamis well supported with ongoing professional development (training support, CPD,and peer support). The role contributes to the improving quality of care of ourpatients across the network of practices.

1.Registered with the General Medical Council (GMC) obtained the Certificate of Experience from the Joint Committee on Postgraduate training for General Practice 3.Membership of appropriate Medical Organisation (MDU, MPS, etc) appraisal updates
