We are currently looking for motivated and enthusiastic Healthcareassistants to join our community / District Nursing teams. The role involvesproviding professional and holistic nursing care to patients in their own homesor care setting.

We have various posts across the teams within County Durhamand Darlington at the moment, with full time and some part time positionsavailable.

localities covered are:

Sedgefield Ferryhill and Spennymoor

Shildon and Aycliife

Overnight District Nursing Team - Countywide

Main duties of the job

Therole involves providing holistic nursing care to patients in their own homesand other environments such as 24 hr residential care. A wide range of nursingskills are required for the role, including but not limited to: venepuncture,wound care, administration of insulin, clinical investigations samplecollection, ECGs. Training and supportcan be given to gain and further develop skills.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job description

Job responsibilities

You will provide holistic care forpatients in their own homes and other community settings, such as residentialcare homes. You will be working as partof the District Nursing team but will be expected to work alone with supportfrom the wider team.

You will need to travelindependently by car so a full UK driving license is required for this role.

For the Aycliffe and Shildon Teamand Ferryhill and Spennymoor Team you will be expected to work between the hrsof 08:00am 21:00pm, we will offer various shift patterns which can benegotiated. Independent working will berequired.

The Overnight Team you will beexpected to work night shifts, the hours of this are 21:00pm 08:00am this isnon-negotiable. You will be workingalongside a RN overnight and be expected to drive long distances due to theareas we cover.

You will not be expected to work days and nightsand will be asked in interview which role would be your preference.

Person Specification

Personal Attributes

Essential

Good interpersonal skills. Able to engage and build effective relationships at all levels. Understanding of the role of HCSW. Able to work as part of a team. Knowledge and understanding of the need for confidentiality.

Qualifications

Essential

Relevant NVQ Level 3 or equivalent relevant experience. Care Certificate or willingness to complete within first 6 months of commencement in post. GCSE equivalent in English and Mathematics.

Desirable

Short courses relevant to undertake patient and clinical care duties.

Experience

Essential

Computer skills. Ability to learn new skills.

Desirable

Previous experience in caring for others.

Special Skills and Knowledge

Essential

Good verbal and written command of English. Ability to act upon own initiative. Good communication skills. Able to work under some pressure. Ability to show leadership. Ability to plan own work. Ability to work calmly under pressure. Willing to take and record patient observations.

Desirable

Knowledge of care and related procedures, clinical observations, relevant legislation.

Self-motivated and able to motivate others. Excellent communication & interpersonal skills. Reflective practitioner. Ability to work in a busy and varied work environment and respond to change.

Be able to offer pastoral care to mentors and students when providing feedback on performance.
