Job overview
Working Full Time: 37.5 Hours Per Week
To work in conjunction with the Clinical Site Coordinator in providing an efficient and effective access point for all patient referrals to the Trust from a variety of internal and external sources.
To support the clinical site co-ordinators in bed management and effective patient flow, whilst maintaining access targets.
To provide a high standard of clerical support and information to the clinical teams, utilising the Teletracking system to enable proactive patient movement through the Trust.
Main duties of the job
Receive emergency admissions via phone from GP'sUpdate CSC's with potential discharges.Attend all wards pre/post board rounds and ensure tele tracking is up to date.Communicate with discharge coordinators based on wardsMaintain numerous live streams teams lists.Communicate with porters and bed turnaround teamsAttend and report the bed capacity meeting three times day.Working for our organisation
