Job summary

Paramedics work autonomously within the 4 GP surgeries that make up our PCN using enhanced clinical assessment and treatment skills, to provide first point of contact for patients presenting with undifferentiated, undiagnosed problems relating to minor illness or injury, abdominal pains, chest pains and headaches. We are looking for someone who has the capability to make sound judgements in the absence of full information and to manage varying degrees of risk when there are complex, competing or ambiguous information or uncertainty.

We are looking for a Paramedic who has completed their two year consolidation of learning and has a further three years experience as a band 6 (or equivalent) paramedic. To clarify, this means you will be required to have 5 years post registration experience.

Main duties of the job

History taking,physical examination, problem-solving and clinical decision-making to establish a diagnosis andmanagement plan.

To provide assessment, treatment and diagnosis at point of first contact by attending to patients ina variety of clinical or non-clinical settings according to patients needs.

To undertake home visits in accordance with the relevant protocols.

To refer patients to an alternative care setting or treat and discharge as appropriate.

Advise patients on general healthcare and promote self-management where appropriate, includingsignposting patients to other community or voluntary services, PCN services and to self-care.

Perform specialist health checks and reviews.

Perform investigatory procedures as required.

Undertake the collection of pathological specimens including intravenous bloodsamples, swabs, etc.

Perform investigatory procedures needed by patients and those requested by GPs and the wider PCN.

Support the delivery of anticipatory care and end of life care plans and lead certain community services ( monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing).

Communicate at all levels across PCNs and other organisations, ensuring effective, patient-centred service. Communicate proactively and effectively with all colleagues across the multidisciplinary team, attending and contributing to meetings as required and accepting referrals and referring to specialist services where appropriate.

Job responsibilities

1. Assess and triage patients, including same day triage, and as appropriate providedefinitive treatment or make necessary referrals to other members of the PCN orpractice team.

2. Advise patients on general healthcare and promote self-management whereappropriate, including signposting patients to other community or voluntary servicesand to self-care.

3. Be able to:

i. perform specialist health checks and reviews

ii. perform and interpret ECGs; alongside other results as appropriate

iii. perform investigatory procedures as required

iv. undertake the collection of pathological specimens includingintravenous blood samples, swabs, etc.

v. perform investigatory procedures needed by patients and thoserequested by GPs and the wider PCN

vi. perform a domicillary (home) visit

4. Support the delivery of anticipatory care and end of life care plans and lead certaincommunity services ( monitoring blood pressure and diabetes risk of elderlypatients living in sheltered housing).

5. Provide an alternative model to urgent and same day home visits for the PCN andundertake clinical audits.

6. Communicate at all levels across PCNs and other organisations, ensuring effective,patient-centered service.

7. Communicate proactively and effectively with all colleagues across the multidisciplinary team, attending and contributing to meetings asrequired and acceptingreferrals and referring to specialist services where appropriate.

8. Prescribe, issue and review medications as appropriate following policy, patient groupdirectives, NICE (national) and local clinical guidelines and local carepathways. This willbe done in accordance with evidence-based practice and national and practiceprotocols, and within scope of practice relevant if possesses prescribingqualification only.

9. Assess, diagnose, plan, implement and evaluate treatment and or interventions andcare for patients presenting with an undifferentiated diagnosis, and patients withcomplex needs within the clinical and domiciliary setting.

10. Clinically examine and assess patient needs from a physiological and psychologicalperspective, and plan clinical care accordingly

11. Prioritise health problems and intervene appropriately to assist the patient in complex,urgent or emergency situations, including initiation of effective emergency care.

12. Take an active part in achievement of contractual markers.

13. Implement and participate in vaccination programmes as appropriate.

Qualifications

Essential

We are looking for a Paramedic who has completed their two year consolidation of learning and has a further three years experience as a band 6 (or equivalent) paramedic. To clarify, this means you will be required to have 5 years post registration experience.Ability to manage and prioritise workload. Ability to work flexibly in core hours to meet the needs of the service. Proven ability to work autonomously and as part of a team. Sound organisational skills. Excellent communication skills. Able to professionally and appropriately challenge differing opinions as a patient advocate. Experience of working within a multi-disciplinary team.
