About the role

Location: Community based in Chapel Town and Harehills initially, with some work across Leeds expected. Elements of homeworking.
As a Dementia Adviser you will have the rewarding opportunity to provide support and guidance to people with dementia and their carers; helping to maintain their independence, improving their sense of well-being, and putting them in more control of their lives. The service is unique to each service user as it is based upon their personal circumstances and support needs.

You will offer support and guidance to your clients in a variety of ways, whether in the client's own home or at other locations, face to face, by phone, letter, or email. You will assist service users to access other services, providing signposting and referrals. Your ability to provide person centred assessment of need, along with a clear understanding of the need for client confidentially is essential to providing a valuable and worthwhile service.

You will work from an integrated team base and from home across a defined area of Leeds. You will need to access to a Wi-Fi connection at home. You will also need to be able to travel independently across your area.

About you We are looking for an exceptional individual, with an understanding of memory loss, dementia and the needs of those affected with these conditions. Your ability to assess client needs along with the understanding of the need for client confidentially is essential to providing a valuable and worthwhile service.
