We are looking for a dynamic individual to jointhe Darlington Community Therapy Team working primarily within our Stroke andNeuro Team alongside Physiotherapists, Occupational Therapists, Speech andLanguage Therapists and Therapy Assistants . This post will include independentworking in a community setting.

Main duties of the job

The post holder will predominantly work independently in acommunity setting with adults referred to the team who present with a range ofneurological dysfunctions resulting from a stroke or a neurological conditionas well as dysfunction resulting from injury, illness, surgery or chronicdisease.

You will be responsible for independently managing acaseload of non complex patients, completing assessments independently, settingshort term therapy goals and treatment plans, completing timely reviews anddischarging patients from the service.

You will develop manual handling and therapy skills to workwith patients with neurological conditions in a variety of settings, both inour gym and in the community alongside our neurological therapists.

The successful candidate will be expected to completedocumentation at point of care and be able to use a lone worker system incompliance with Health and Safety.

The post holder will attend weeklymulitdisciplinary team meetings to assist in the management of the complexpatients with multidisciplinary needs

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job description

Job responsibilities

To plan and organise clinical activity, with appropriatetime scheduling in order to complete all clinical and administrative dutiesincluding statistical record keeping.

To delegate treatment tasks to therapy assistants

To actively engage and be responsible for own supervisionwith a senior member of staff.

To be responsible for clinical record-keeping in accordancewith the guidelines for documentation and legal requirements.

To be responsible for the correct and safe use of thephysical environment including equipment and to be responsible for theallocation of these resources.

When working in the community setting, to work as a lonepractitioner with telephone support from a Senior Therapist if required.

To ensure all mandatory training is kept up todate and participate in professional development including attending In ServiceTraining.
