Job summary

An exciting opportunity has arisen for a proactive and dynamic candidate to provide operational leadership and line management to the Community Access Team, who are based at the Norfolk and Norwich University Hospital.

The Team lead will manage the workforce, take responsibility for quality monitoring and work collaboratively with system partners.

The successful candidate must be able to make own travel arrangements.

Main duties of the job

To be responsible for the day-to-day operational line management of the team, leading and ensuring a 7 day 8am-8pm service is provided. To facilitate the discharge of patients to community hospitals and procured beds working within statutory timeframes and operational targets To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users

About us

The Community Access Team has 2 elements. The front door team at the Norfolk and Norwich University Hospital consisting of Occupational Therapists, Physiotherapists and Assistant Practitioners providing an Admission Avoidance service. Working hours of 8-8, 7 days a week.

The back door of the Community Access team is provided by Nurses at the Norfolk and Norwich Hospital who support with the review and transfer of patients from Norfolk and Waveney acute hospitals to our 7 inpatient Community Units Across Norfolk. Working hours 8-6 Monday to Friday and 8-4 weekends/bank Holidays.

Job responsibilities

To be responsible for the day-to-day operational line management of the Team. To plan, coordinate and evaluate the Services and ensure they are safe, efficient and effective with regard to patient management and use of staff time. To facilitate development of the integrated team and individual staff members to promote job satisfaction, performance and an ability to manage change. To actively perform absence management, ensure absence reporting is undertaken in a timely manner, as per the Trusts Policy, working closely with HR. Deputise for the Integrated Service Lead as required Work collaboratively with healthcare professionals and statutory / non-statutory agencies to provide a seamless, integrated service to our service users. To adapt communication style to meet needs of patients, staff and colleagues, using tact, diplomacy and sensitivity including overcoming community barriers To develop and maintain strong, transparent working relationships with key stake holders To monitor and manage the Team budget To propose and Implement departmental policy changes for the management of patients referred to the service, networking with other specialist practitioners To represent the service and NCH&C at strategic meetings with partner organisations as requested To manage the Community Access Team caseload Person Specification

Qualifications

Essential

Registered Nurse/Therapist First level degree, diploma or equivalent Current registration to a professional body Recognised management qualification or evidence of working at that level

Masters level degree

Experience of leading a team Experience of working in a multi- agency environment Experience of managing budgets A track record of successful service and policy development and implementation Performance Management Experience Evidence of post-graduate development
