Job summary

An exciting opportunity has arisen to join our friendly and innovate Primary Care Network (PCN) Poole North as we seek an Advanced Nurse Practitioner to work across two of our four practices to help support the acute, on-the-day need of our patients, both in surgery and on home visits.

The ideal prescribing ANP candidate will manage a daily caseload and deal with a wide range of health needs across our modern and spacious practices ensuring the highest standards of care for all registered and temporary patients and will either have their masters degree and/or their Digital Badge in place.

Main duties of the job

The post-holder will:

act as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients attending within primary care initiating direct referrals as appropriate in a variety of appointment types (face to face, telephone, video. Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence. Undertake clinical nursing practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary health care services. Assesses diagnoses and treat patients attending primary health care services with a range of acute, non-acute and chronic medical conditions. Assess diagnoses and treat patients in surgery who require acute medical attention and refer appropriately in line with local and national guidance. Admit patients to secondary care for acute medical need. Refer patient as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations including referral for x-ray in line with policy and appropriateness. Order blood, x-ray and other pathology testing via ICE. Make critical judgements to satisfy the expectations and clinical need of the patient, where appropriate. Prescribes independently in line with the formulary.

About us

Poole North Primary Care Network is an innovative, friendly and supportive PCN of four GP practices who work well together in providing services for our 52,000 patients. By providing additional ANP support in two of our practice locations we are aiming to ensure patients have better access to GP services. The successful applicant will work alongside our existing ANP in Hadleigh and Birchwood and although the roles are separate and working within different practices a counterpart in the network can help foster team working.

The successful candidate will additionally have supervision regularly from a GP supervisor.
