Job summary

Do you want to take your career tothe next level with support to develop, and lead our enthusiastic, fun and skillmixed team pharmacy team? The role of the Senior Clinical Practice Pharmacistis well embedded in the wider practice clinical team and the post would suitthose ideally with primary care experience looking for the next challenge.

You would be working solely for AirBalloon Surgery and join our existing team of experienced prescribing ClinicalPractice Pharmacist and Pharmacy Technician.

You will be regularly supervised by aGP partner and will receive opportunities to grow and develop professionally.

The post could be worked as full timeover five days, or part time - minimum of 34 hours over four days.

Competitive salaryoffered, negotiable depending on experience and skill set. weeks pro rata annual leave per year with one week study leave

We see this post as a senior clinicalrole within the wider clinical team and are keen to embrace and develop thepotential for clinical pharmacy in our practice vision for high qualityclinical services.

The successful candidate will have aminimum of five years post graduate experience and ideally be an independentprescriber with primary care prescribing experience, although we would supportan exceptional candidate to gain their prescribing qualifications. They willhave completed or will be willing to complete the Primary Care PharmacyEducation Pathway.

Main duties of the job

Leadership of the Practice Pharmacy Team- ensuring the team delivers acomprehensive high-quality service to the patients and staff of the practice

Patient facing roles seeing patients and managing medication relatedproblems and queries. Medication reviews at point of care transfer, especiallyfor older frail, care homes and polypharmacy

Staff facing roles - providing support to general practice staff inrelation to repeat prescriptions and medication queries

Quality, governance and improvement - working with the prescribing andwider practice team on safety and medicines optimisation. Leading oninitiatives, medicines audit work and local projects and schemes of work

Job responsibilities

The post holder is an experiencedclinical pharmacist with previous experience of working in Primary Care, whoacting within their professional boundaries and as part of the wider clinicalteam, will develop and manage medicine management services within the practice.

Thepost holder will lead the multi-disciplinary pharmacy team, to meet practiceservice requirements for medicines management.

Thepostholder will manage systems and be part of the workforce relating to repeatprescribing, authorisations and reauthorisation, acute prescription request,management of medicines on transfer of care and systems for safer prescribing.

Thepost holder will lead processes and perform medication review of patients withpolypharmacy especially for older people, people in residential care homes andthose with multiple co-morbidities.

Thepost holder will provide leadership on medicines optimisation, leading on localICB projects and schemes of work, quality improvement and clinical audit.

Experience

Essential

Minimum of 5 years post qualification experience; In-depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare; Gain acceptance for recommendations and influence/motivate/ persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers; Demonstrates accountability for delivering professional expertise and direct service provision;

Previous line management experience;

Qualifications

Essential

Mandatory registration with the General Pharmaceutical Council (GPhC) Masters degree in pharmacy (MPharm) (or equivalent) Independent prescriber or working towards/intent of gaining an independent prescribing qualification

Desirable

A member of or working towards Faculty membership of the Royal Pharmaceutical Society Specialist knowledge acquired through post- graduate diploma level or equivalent training/experience

An appreciation of the nature of GPs and general practices; An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing; Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences ( patients); Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions; Good IT skills; Able to obtain and analyse complex technical information; Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate;
