We are looking for a kind and fun-loving person to join our amazing team in Great Finborough, supporting 5 people who live independently. You will be working in peoples homes, so its important you can connect, share interests and be interested. The people we support love to get out and about using trains and buses and boat watching. As well as daily walks, exploring their wider community and socialising with their friends and family.
So if you enjoy building meaningful relationships and think you could plan, communicate and deliver support with and alongside people, then this is the place for you.
Main duties of the job
You should be available to work a variety of shifts across the week including some long days, some weekends and sleep-in shifts as needed. We have Full Time and Part Time positions available.
An essential part of this role is being a driver and having access to your own vehicle due to its rural location.
What will you do?
Supporting people is really important but its not all trips out and artwork. We also support people to take their medication, make healthy lifestyle choices or maintain their home in a way they are safe and well.

Job Purpose
To support people with learning disabilities with all aspects of their daily lives, ensuring thatthey have maximum opportunities and choice for developing new skills and leading fulfilling,varied, are valued and live socially inclusive lifestyles where their rights, dignity and safetyis protected.
Key Responsibilities
1. To identify and develop with each person an outcome based support plan, riskassessment and person centered plan which are reviewed and updated regularly
2. To assist the people we support with routine personal and domestic tasks, includingpersonal care, household tasks and cleaning and promote a healthy lifestyle whenshopping, meal planning, cooking and choosing activities
3. To monitor and promote each person's health and well being, ensuring that eachperson has a Health Action Plan, that medical help is sought when needed, dental,eye, hearing and other check-ups or treatment are undertaken and that changes inhealth and wellbeing are reported
4. To assist the people we support to participate in their chosen social, educational,leisure and sporting activities in the local community, taking into account theirculture, faith, gender and sexuality and where desired, to support individuals toaccess volunteering or employment opportunities
5. To assist the person to improve his/her knowledge, skills and abilities throughplanned learning activities and experiences
6. To enable the people we support to keep and make new friends and keep in contactwith family, friends and people in the neighbourhood and develop a circle of support
