Main duties of the job

The assistance practitioner will work closely with the team to support the community rehab team to assess, plan and implement a range of therapeutic activities and treatments to improve functional ability, mood and the well-being of patients . The role involves supporting people who have been recently discharged home as well as those who are already at home and require rehab at home.

The post holder will be responsible for activities delegated to them and will work without direct supervision but will have access to a clinician for advice and direction as required.

Assistants are expected to continually improve their knowledge and skills and develop collaborative working relationships with therapy colleagues to enhance patient care. Duties are varied and include direct patient contact, clerical and housekeeping tasks.

The post holder will be required to have completed a foundation degree or equivalent in health and social care, and understand the requirements of rehabilitation and have experience of working with people with a range of health needs.

The post holder needs to be able to access a range of community bases and travel to patients homes in what can be very rural areas. A driving license and access to a vehicle for work purposes is essential .

Qualifications

Essential

Good general level of education including GCSEs in English and Maths at grade C or above Relevant Foundation degree, or BTEC higher or QCF level V qualification in health and social care

Desirable

4 GCSEs Grade A - D Courses with Relevance to the job equipment, dementia ECDL or formal qualifications in word processing and spreadsheets

Experience

Essential

Recent, relevant experience of working in therapy services Experience of working with people with complex health needs Clinical experience in acute, mental health or other clinical support worker roles. Experience of carrying out therapeutic interventions Experience of multidisciplinary team working Experience of working independently Experience in use of IT applications

Desirable

Experience of carrying out therapy Experience of project work Experience of working shifts/ weekends Experience of supervising and supporting others Evidence of therapy competencies

Knowledge

Essential
