Job summary

Lincolnshire Community Health Services (LCHS) are working in partnership with the acute trust to provide a new and exciting opportunity for any registered qualified practitioner interested in delivering an IV service to patients in their own homes or community settings.

This job is open to all registered nurses who are able to deliver or train to deliver IV Therapy.

A bespoke training package has been developed where you will work closely with members of the IV team as well as the outpatients and day theatre teams

The job will cover 7 days working, full and part time posts will be considered.

This teams aim will be to provide care to the patients within their own homes or community settings to help ease bed capacity within the acute hospitals and to deliver a care closer to home for their patients.

The new adventure is due to start as soon as possible and will involve a small team of registered practitioners working closely together to deliver patients IV Therapy, mainly antibiotics, to the patient within the comfort of their own homes or communities.

Teams are based in three community hospital sites across the county, Gainsborough, Skegness and Spalding.

Main duties of the job

The post holder will be responsible for the assessment ofperson-centred care needs, and lead on the implementationand evaluation of the OPAT and IV therapy will be delivered to a high standard in any health andcare setting that includes the person's home, the communityor hospital or in any health care setting where their needsare supported and managed in collaboration with themultidisciplinary lead a team that drives forward innovations andimprovements in this area.

Main responsibilities

To deliver IV Therapy to patient within their own home or community setting

To be able to assess and manage CVAD lines

To be able to record electronic patient records

To be up to date with safeguarding and life support training

Demonstratingappropriate prioritisation, delegation and assignment ofcare responsibilities to others with the organisation ofpatient visits, as well as with effective supervision andevaluation of the team and patient care deliveryTo work as an accountable and autonomous practitionerin the assessment, planning, delivery and evaluation ofsafe and effective person-centred nursing care, withparticular focus on IV therapy and prevention of hospitaladmissions. Developing and implementing specialistprogrammes of care ensuring theory relating to be able to communication sensitive, confidential andcomplex information concerning the patients diagnosis,treatment and state of well-being this requires empathyand reassurance. Patients could be hostile at patients may have a barrier to theircommunication such as English not their first language. To develop, maintain and utilise key clinical skillsrequired to ensure delivery of the OPAT and IV therapyservice in community and in-patient settings. Working inpartnership and collaboratively with multidisciplinaryteam members and statutory and voluntaryorganisations in response to individuals health, social,psychological and emotional all documentation relating to patient careacquired by themselves is accurate, legible and safelystored as per trust manage own workload as well as providing day today supervision as well as clinical supervision to all staffor students when necessary.Undertake surveys and audits as necessary for service,participating in the implementation of policies andproposing any changes to practice or procedures withintheir work place if necessaryTo provide and receive sensitive, complex andconfidential information concerning patients and staff.Sometimes hostile, and within a highly identify and report any work-related risks inaccordance with Trust policy, utilising the appropriatereporting mechanisms, co-operating with those withdesignated responsibility in the investigation andmanagement of identified take all reasonable care for own health and safetyand that of others, by ensuring equipment is used safely,is well maintained and in good working order.
