Job summary

The nurse associate role in Sowe Valley PCN is an exciting and innovative role where you will have access to an unrivalled range of training and development opportunities and work with our experienced multi-disciplinary practice and PCN teams.

You will be a registered Nursing Associate and will have completed your Foundation Degree.

You will work as part of a Primary Care Network multidisciplinary team, providing safe, person-centred care to improve the health of our population.

Main duties of the job

The nursing associate role is varied and includes:

Vaccinations and immunisations Venepuncture (blood tests) ECG recording Cervical screening (full training provided) Learning disability health checks NHS health checks Long term condition reviews A range of treatment room duties including dressings, removal of sutures etc.

Full training, supervision and mentorship will be provided through our excellent links to local universities and training providers and experienced nursing teams.

You will work in collaboration with a range of other professionals including GPs, practice nurses, district nurses, and other community services.

About us

Sowe Valley PCN is a network of ten GP practices in the East of Coventry with 84,000 registered patients and a track record of innovation and delivery.

We are conveniently located close to the M6 and M69 road networks with excellent rail links from Coventry Station.

We have established a multi-disciplinary team of additional roles to integrate with and support our practice teams including:

Paramedics Clinical Pharmacists Pharmacy Technicians Nurse Associates Care Coordinators Social Prescribers First Contact Practitioners (Physiotherapists)

We have led a COVID vaccination centre delivering over 170,000 vaccinations from the Coventry and North Warwickshire Cricket Club, the highest performing PCN in our local area.

Sowe Valley PCN is an innovative, responsive network established to support the delivery of high quality, integrated care for our patient population.

We are currently leading a population health management programme for our STP, working in partnership with UHCW, CWPT and public health to use population health data to shape health and social care delivery and joined up care.

As a Nursing Associate, you will work under the supervision of and in partnership with the Senior Nurses to provide and deliver a high standard of nursing care. You will contribute to the on-going assessment, providing and monitoring care based on evidence and working in partnership with the multi-disciplinary team.

Key responsibilities

Provide compassionate, safe and effective care and support to patients To safely undertake the following procedures to meet the patients nursing needs to include; venepuncture, ECG recording, recording monitoring, vital signs, wound care Carry out immunisations in accordance with local and national PGDs/PSDs/Protocols Provide general and specific health screening, including cervical cytology and NHS health checks Perform learning disability health checks liaising with the wider MDT as appropriate. Prioritise and manage own workload, recognising where elements of care can safely be delegated to other colleagues, carers and family members. Provide and receive sensitive information concerning patients medical conditions. Work with registered nurses and other professionals and engage with other agencies and community teams to ensure a safe, personal approach for patients. Maintain accurate, contemporaneous patient records in line with practice policy and the NMC Code. Provide effective evidence based care for service users under the supervision of registered nurses and other professionals in order to perform non-complex activities, in line with clinical governance and professional standards. Qualifications

Essential

Nurse Associate Foundation Degree GCSE grade A to C in English and Maths (or equivalent)

Desirable

Qualified in phlebotomy Qualified in immunisations and vaccinations Qualified in cervical screening

Experience

Essential

Experience of working within a nursing team Experience of dealing with vulnerable patients Experience of working is a busy environment Experience of working autonomously within scope of practice
