Coventry Specialist Palliative Care Team provides Palliative Care for people with an advanced progressive illness in order to improve quality of life for patients and their families. The team strive to work in a multi-disciplinary, integrated way to offer the best possible care. We regularly undertake audit/surveys and are involved in national research.
This role is predominantly managing palliative care emergencies but does also involve unplanned urgent community nursing care as well, such as blocked catheters etc
PLEASE ALSO BE AWARE THAT WE ARE CURRENTLY UNDERGOING TRANSITION FROM CWPT TO UHCW MANAGEMENT, BUT THIS STILL WILL REMAIN A COMMUNITY ROLE

In order to support the end of life pathway and ongoing Out of Hospital work streams we have identified a need for band 5 nurses with palliative skills to support rapid/urgent palliative care into the service 24/7 and co-ordinate a rapid response to patients in their own homes and care homes.
