This is a secondment post to cover Maternity until 18-04-2025

Main duties of the job

Your role will centre on the delivery of specialist mental health support to people with severe and enduring mental illness who are requiring treatment and care in the community.

There will also be a requirement to complete statutory social work duties on behalf of the local authority including the completion of Care Act Needs Assessments, Mental Capacity Act assessments and to provide social work support to people subject to the Court of Protection

This will be in the form of direct work and care coordination with people on caseload, providing duty support to those under the team and contributing to multi-disciplinary team meetings both in house and with colleagues externally. As a social worker you will ensure this work is undertaken in a strengths focused manner and from a rights based perspective.

Empowering and supporting service users to make effective personal decisions which promote their psychological wellbeing and recoveryUndertakes supported person led assessments, risk assessments, support planning and outcome focused reviews of service users and carers who have eligible needs and complex and/or long-term conditions in accordance with legislative requirements, policy and professional frameworks.

In addition to undertaking person led assessments, you will manage a caseload with complex needs and provide case load guidance, support and specialist advice to junior members of staff and ensure care is completed within the practice and performance framework.

Experience

Essential

Appropriate knowledge/experience of working in mental health/learning disability service areas Be able to demonstrate a commitment to safeguarding and promoting the welfare of vulnerable adults

Desirable

Evidence of promoting/supporting active user/carer involvement/participation Experience of the assessment process in relation to the Care Act

Qualifications

Essential

Degree and current professional registration held with Social Work England Willingness to complete ASYE within first 2 years in post

Desirable

Completion of ASYE Full understanding of relevant policy drivers and their application within the service area

Personal Attributes

Essential

Able to demonstrate effective inter- personal/communication skills. Evidence of good time management skills Be able to demonstrate core values of social work Able to demonstrate ethical values and attitudes within a culture of equality and diversity

Desirable

Working knowledge of Trust policies and procedures.
