Job summary

This is a great opportunity for a qualified Social Worker, Mental Health Nurse or Occupational Therapist to work collaboratively between SHSC and Sheffield City Council (SCC) Children, Young People & Families (CYPF) service. The role is commissioned to work with families, with the overall aim to improve outcomes for the whole family.

The service aims to increase the knowledge and understanding of staff who work with Children, Young People & Families to improve their skills in identifying and supporting adults with mental health issues and determine thresholds to specialist services.

The role also aims to improve integrated working between services, exploring and improving referral pathways, and overall integrated family working with adult mental health services.

This is a fixed term contract or a secondments opportunity until March 2025, but is envisaged to be continued. We are open to secondments from Local Authority employees pending line manager approval.

This post involves both home working and working in various bases, throughout the City dependant on service need.

Main duties of the job

Increase the understanding and improve the skills of staff who work with children and families around adult mental health issues and thresholds for referral to specialist services.

Improve integrated working between services, exploring and improving referral pathways as necessary.

Reduce the number of inappropriate referrals into adult mental health services by upskilling the Children, Young People and Families (CYPF) workforce.

Work proactively with all services involved with families to increase their confidence, knowledge, understanding and skills in supporting adults with mental health issues.

Provide consultations to workers regarding adult mental health issues within families who have a allocated Children, Young People & Families (CYPF) service workers.

Contribute to and have a role in the whole family assessment and whole family action plan.

Signposting the adult to appropriate support, both statutory and third sector services.

Offer initial mental health assessment of adults where further clarity is required.

Offering the adult a brief intervention if deemed the most appropriate action.

Carry out home visits if and when necessary.

Contribute to service development.

Identify when common themes occur and develop targeted training and group work where necessary and support the delivery of such.

Deliver agreed training and group work to staff and service users.

Produce newsletters and case studies when necessary to share knowledge and upskill the workforce.

Job Purpose

The post holder will work collaboratively with the Family Intervention Service (FIS) workers within Sheffield City Council (SCC) Children, Young People & Families (CYPF) service and those services commissioned to work with families, with the overall aim to improve outcomes for the whole family. This project aims to increase the knowledge and understanding of staff who work with children and families to improve their skills in identifying and supporting adults with mental health issues and determine thresholds to specialist services. The role also aims to improve integrated working between services, exploring and improving referral pathways, and overall integrated family working with adult mental health services.

General Requirements

To assess, evaluate, assist and advise in the management of clinical risk. To formulate a plan of care based on identified need. To deliver evidence based interventions as appropriate. To work collaboratively with service users, carers and partner agencies. To maintain computer held care records in an accurate and timely fashion. Undertake all work in a culturally sensitive manner. Develop effective working relationships with colleagues from varying agencies.

Service Development

Supervision

Regularly review cases with supervisor to promote efficacy and contribute to the overall management of the Team Around the Family. Regularly review workload, including training, groupwork and case work. Participate in own PDR process, identifying training and development needs, and pursuing relevant actions to maintain and improve level of competence and effective contribution.

Education and Training

Ensure you are compliant with SHSC mandatory training requirements. Participate in training relevant to role. Promote insight and understanding of mental health issues to service users, carers, and other agencies. Contribute to audit, research and evaluation as appropriate. To be involved in delivering training for staff in Children, Young People and Families (CYPF) in line with the Early Help offer. To be involved in delivering groupwork to adults.

General

Maintain professional registration and adhere to professional guidelines and code of conduct. Work within the framework of relevant policy and legislation both local and national.

Duties may be modified as a result of identified development needs and/or in respect of changing demands upon the service. Any changes will be discussed with the post holder prior to implementation.

Job Description Review

This is not an exhaustive list of duties and responsibilities and in discussion with the manager the postholder may be required to undertake other duties which fall within the Banding of the post. The job description will be reviewed regularly in the light of changing service requirements and any such changes will be discussed with the postholder.

The post holder should be comfortable in the use of computer technology, have basic keyboard skills and have the ability to use email.

Staff are expected to comply with the Trusts Infection Prevention & Control policies and conduct themselves in such a manner as to minimise the risk of causing healthcare associated infections.

Knowledge and Skills

Essential

Working knowledge and understanding of adult community mental health services Knowledge and understanding of therapeutic approaches relevant to role Knowledge and understanding of PDR and supervision processes as supervisor and supervisee Demonstrate skills in risk assessment and risk management Demonstrate an awareness and understanding of legislation and other factors influencing mental health services Ability to assess, plan and evaluate care Ability to work both autonomously and as an effective team member of a multi-agency team with an ability to communicate at all levels. Ability to maintain accurate and contemporaneous client records and data utilising the various electronic recording systems in SHSC and CYPF. Demonstrate caseload and time management skills. Ability to deliver / facilitate training sessions to staff and group work to clients.

Desirable

An understanding of CBT (Cognitive Behavioural Therapy) approach.
